Clinical Observation Of Shenqi Dih Uang Decoction Combined With Tacrol Imus In The Treatment Of Membranous Nephropathy With Deficiency Of Qi And Yin And Blood Stasis

Objective:To observe the clinical effect of Shenqi Dihuang Decoction com bined with tacrolimus in the treatment of membranous nephropathy with def iciency of Qi and Yin and blood stasis.Methods:A total of 80 patients with membranous nephropathy conforming to the syndrome of Qi and Yin deficiency and blood stasis were randomly divi ded into control group and treatment group,with 40 cases in each group.The control group was given tacrolimus and western medicine basic treatme nt,the treatment group was given Shenqi Dihuang decoction on the basis o f the treatment of the control group,the course of treatment was 6 month s.Clinical symptoms of TCM were observed,and changes of urea nitrogen(BUN),serum creatinine(SCR),serum cysteine protease inhibitor C(CysC),serum albumin(ALB)、C-reactive protein(CRP),serum anti-phospholip ase A2 receptor(PLA2R)antibody,24-hour urine protein quantitative(UTP)and other laboratory indexes were recorded.Collect data and conduct sta tistical analysis.Results:1.There were no statistically significant differences in gender distribution,age distribution,pathological stage,laboratory indicator s before treatment and TCM syndrome scores between the treatment group an d the control group(P>0.05),indicating comparability.2.Clinical eff icacy:the total effective rate of the treatment group was 89.7%,and the total effective rate of the control group was 69.2%,P=0.025<0.05,ind icating that the treatment group was better than the control group.3.TC M syndrome score:After treatment,the total TCM syndrome score decreased in both groups(P<0.05),and it was more significant in the treatment group.4.Urea nitrogen and serum creatinine:After treatment,the condit ions of urea nitrogen and serum creatinine in both groups were significan tly improved(P<0.01),and the improvement in the treatment group was b etter than that in the control group(P<0.05).5.Serum albumin:After treatment,serum albumin increased in both groups compared with before tr eatment(P<0.01),and the effect in the treatment group was more signif icant(P<0.01),the difference was statistically significant.6.24h uri nary protein quantification:24h urinary protein in both groups was decre ased compared with before(P<0.05),and the effect of the treatment gro up was significantly higher than that of the control group.7.Serum cyst eine protease inhibitor C and C-reactive protein:both groups were decrea sed before and after treatment(P<0.01),indicating that the effect of the treatment group was significantly higher than that of the control gro up(P<0.01),with statistically significant differences.8.Anti PLA2R a ntibodies:After treatment,the anti PLA2R antibodies level in both group s was lower than before(P<0.05),and the effect in the treatment group was significantly higher than that in the control group.9.Side effects:there were statistically significant differences in infection between th e two groups(P<0.05).There was no significant difference in hypertens ion,elevated blood glucos(P>0.05).Conclusion:1.Shenqi Dihuang Decoction combined with tacrolimus has obvi ous curative effect in the treatment of membranous nephropathy with Qi an d Yin deficiency and blood stasis,and the combined effect of Shenqi Dihu ang Decoction combined with tacrolimus is better than that of tacrolimus alone.2.Shenqi Dihuang Decoction combined with tacrolimus can improve t he clinical symptoms of membranous nephropathy with Qi and Yin deficiency and blood stasis,reduce the urinary protein,slowly improve renal funct ion,reduce inflammatory injury,increase serum albumin and decrease the level of anti PLA2R antibody.
