Objective:Follow the wound staging management,optimize the modified Sihuang ointment intervention program in the peri operative period of anal fistula,evaluate its clinical efficacy of relieving wound inflammation and preventing wound infection,and embody the unique characteristics of traditional Chinese medicine.Methods:A total of 72 cases of low anal fistula were collected from the Department of anorectal,Baoan Hospital of TCM from May 2020 to January 2021.The treatment group was intervened with modified Sihuang ointment before and after operation,and the control group was intervened with compound polymyxin B ointment after operation.The wound pain,secretion,granulation color and granulation density were observed on the 1st,3rd,7th and 14th day after operation,and surface area reduction rate and longitudinal diameter reduction rate of the wound on the 7th,14th and 28th day after operation were also observed.Results:1.There was no difference in gender,age,course of disease,anal fistula type,operation time,initial surface area and initial longitudinal diameter between the two groups(P>0.05).2.In the aspect of wound pain:there was no difference in VAS score on the first day after operation between the two groups(P>0.05),but on the 3rd,7th and 14th day after operation,the score of the treatment group was lower than that of the control group(P<0.01).3.In the aspect of wound secretion:there was no difference in the score of the first day after operation between the two groups(P>0.05),and the improvement of the treatment group was better than that of the control group on the 3rd,7th and 14th day after operation(P<0.05).4.In terms of granulation tissue color:there was no difference in the score of the first day after operation between the two groups(P>0.05).On the third day after operation,the purulent coating on the wound surface of the treatment group was more than that of the control group(P<0.01),but the improvement on the 7th and 14th day after operation was better than that of the control group(P<0.01).5.In terms of granulation tissue density:there was no difference between the two groups on the 1st and 3rd day after operation(P>0.05).On the 7th and 14th day after operation,the improvement in the treatment group was better than that in the control group(P<0.05).6.In terms of the reduction rate of wound surface area:there was no significant difference between the two groups on the 7th day after operation(P>0.05).On the 14th and 28th day after operation,the reduction rate of the treatment group was better than that of the control group(P<0.01).7.There was no significant difference between the two groups on the 7th day after operation(P>0.05).On the 14th and 28th day after operation,the reduction rate of the treatment group was better than that of the control group(P<0.01).Conclusion:Modified Sihuang ointment is safe and effective in peri operative intervention of anal fistula.It can improve the quality of wound healing by alleviating wound inflammation and preventing postoperative infection,providing a good wound bed environment for granulation tissue growth.
