chapter Ⅰ Anatomical study of medial sural artery perforator flapObjective:By dissecting lower limb specimens,To understand the number、position、type、Vascular pedicle long、 perforators long、External diameter of vessel at pedicle、 The outer diameter of the perforator vessel passing through the deep fascia of the perforator branch of the medial gastrocnemius cephalic artery.Methods : Through skin flaps of 6 Chinese adult lower extremity specimens during simulated surgery,observed The course of the medial gastrocnemius cephalic artery and measure the number、position、type、 Vascular pedicle long、 all perforators long、External diameter of vessel at pedicle、 all the outer diameter of the perforator vessel passing through the deep fascia、distance of The popliteal fossa fold and the posterior midline are shallow from the perforator branch.The measured data were inputted into the computer and analyzed by SPSS15 statistical software.Results :6 cases autopsy hints:A total of 11 musculocutaneous perforating branches of the medial cephalic artery of the gastrocnemius muscle were observed in this experimental specimen,1-3 pieces per piece,1.83 pieces on average,The puncture point of the perforator branch is11.2±2.62cm(5.2-16.8cm)cm away from the popliteal fossa fold and2.83±0.91(1.2-4.0)cm away from the posterior midline.The length of vessel pedicle(A)was 11.4±3.05cm(7.2-16.4cm),The length of the perforator pedicle is 3.0±0.57cm(2.0-4.2cm),.The outer diameter of the pedicle vessel was 1.85±0.33mm(1.6-2.3mm),and the outer diameter of the vessel when penetrating the deep fascia was 0.9±0.19mm(0.6-1.2mm).Conclusion: The number and position of medial sural artery perforator flap were constant,The vascular pedicle is longer,Besides,It has the advantages of ideal anastomosis caliber and no damage to the main blood vessels of lower limbs.The anatomical features required for the design of free skin flaps suitable for clinical use.Chapter Ⅱ Clinical application of medial sural artery perforator flapObjective :By using the medial sural artery perforator flap to repair the wound surface of the limbs To observe the clinical feasibility of the flap.Methods : By following up 7 clinical cases from 2017 to 2019,using medial sural artery perforator flap to repair extremities wounds,All patients were followed up regularly after surgery,and the successful follow-up lasted for 3-12 months,with an average of 8 months.The survival,appearance,sensation and function of the flap were observed.Results :All 7 patients in this group were followed up for an average of 8(3-12)months,7 cases of free medial gastrocnemius head perforator flaps survived well after transplantation and had good appearance after healing,The clinical effect of medial gastrocnemius perforator flap in the repair of extremities was further verified.Conclusion :Dissecting the medial sural artery perforator flap without sacrificing the popliteal artery and gastrocnemius muscle,For the donor area the trauma is relatively small,The donor sites of the postoperative flaps were all primary sutured,One underwent local flap transfer and repair;And the flap is thinner than other perforator flaps,All patients were satisfied with the appearance and function of the affected area after repair,The flap is an ideal choice for thin skin flap to repair the wound surface of limbs.
