| Objective: To compare the difference between endoscopic papillary balloon dilation(EPBD)and endoscopic sphincterotomy(EST)in the treatment of choledocholithiasis.To clarify the importance of Oddi sphincter function protection,and reduce postoperative complications and patients’ pain and burden.Methods: Collected 98 patients with choledocholithiasis treated with EST from January 2015 to December2017,and established the follow-up database,which was set as the conventional minimally invasive group.From January 2018 to December 2018,90 patients with choledocholithiasis treated with EPBD was established the follow-up database,which was set as a functional protection group.The data between the two groups were analyzed to evaluate the clinical practicability and superiority of Oddi sphincter function protection.Results: There was significant difference between EST and EPBD in the incidence of perioperative complications and long-term postoperative complications(P < 0.05).The total length of hospitalization in EST group was significantly longer than that in EPBD group(P < 0.05).There were significant differences between EST group and EPBD group in the incidence of postoperative duodenal papillary hemorrhage,reflux cholangitis,choledocholithiasis and the recurrence rate of choledocholithiasis(P < 0.05).There was no significant difference in the incidence of acute pancreatitis and the rate of stone clearance between the two groups(P > 0.05).Conclusion: 1.Compared with EST,EPBD has a lower incidence of complications,The curative effect of EPBD is also safe and reliable,which is worthy of clinical promotion and applicatio.2.EST may result in irreversible damage of Oddi sphincter’s function,and increase the incidence of long-term complications.Therefore,the sphincter function of Oddi should be protected as much as possible during the surgical treatment of choledocholithiasis. |
