Objective:Through the epidemiological investigation of chronic obstructive pulmonary disease(COPD)among the elderly aged ≥60 years in an island area of Ningbo,we can understand the current situation of the disease,diagnosis and treatment,analyze risk factors,especially the status of undiagnosed and untreated,and analyze related reasons.While improving the rate of COPD disease awareness,it will provide data support for COPD community prevention and control strategies.Methods:The survey target was first selected.The elderly people aged ≥60 years old in the scope of Hepu Town,Xiangshan County,Ningbo City.Then,all participants completed a COPD Screening Questionnaire and underwent a post-bronchodilator pulmonary function test.COPDpositive individuals then completed a questionnaire surveying the status of diagnosis and treatment of COPD,and the reasons for undiagnosed and untreated.Finally,the data was sorted into an EXCEL table,and SPSS version 22.0 software package was used for multi-factor analysis.Findings:1.A total of 1,526 people were surveyed this time,of which 1371 were eventually included in data analysis,and the enrollment rate was 89.8%.The survey data showed that 254 patients were diagnosed with chronic obstructive pulmonary disease,and the prevalence rate was as high as 18.5%.2.Chi-square test showed that sex,age,smoking history,smoking pack-years,cough,and dyspnea were risk factors for COPD in the elderly.And body mass index(BMI),family history of respiratory diseases,and history of biofuel exposure were not risk factors for COPD in the elderly.3.Multivariate logistic regression analysis revealed that age and smoking were independent risk factors for COPD.4.According to the chronic obstructive pulmonary disease screening questionnaire(COPDSQ),the ROC curve related to COPD was screened.When the score of COPD-SQ = 19.5,the Jordan index was the largest,so the score of COPD-SQ was significant for the community primary screening of COPD diagnosis.5.Of the 254 patients with COPD,only 10 had a history of COPD diagnosis.The results showed that the undiagnosed rate of COPD in Ningbo island area was extremely high,as high as96.1%.Conclusion:The prevalence of chronic obstructive pulmonary disease(COPD)was 18.5% in an island area of Ningbo,of which the prevalence of men was 28.9% and the prevalence of women was 8.3%.Most COPD patients were in the early stage of COPD classification.Only 10 people with a history of COPD diagnosis and the undiagnosed rate of COPD was 96.1%.Multivariate analysis showed that smoking and age were independent risk factors for COPD in elderly islanders.When the cutoff value COPD-SQ score was ≥19.5 points,it could be used for community early warning of COPD disease and improve the diagnosis rate of COPD.
