Objective This study intends to investigate the correlation between blood selenium concentration and brain atrophy and cognitive function in patients with chronic alcoholic encephalopathy,to provide new ideas for the clinical diagnosis and treatment of chronic alcoholic encephalopathy,and to provide basis for subsequent research.Methods(1)A total of 100 male patients who were admitted to the The Central Hospital Of Enshi Tujia And Miao Autonomous Prefecture and Enshi Center Hospital between July 2019 and November 2020 were selected as the main clinical manifestations consisting of limb tremor,visual,auditory,tactile hallucination,hyperactivity and other clinical symptoms.The mean blood selenium concentration of the study subjects(Se = 70.72 μg/L)was taken as the cut off value,and the study subjects were divided into relatively low blood selenium group(Se < 70.72 μg/L,n =50),relatively high blood selenium group(Se ≥ 70.72 μg/L,n = 50).(2)All samples were subjected to linear measurement of brain atrophy and Mo CA cognitive function score;all samples were tested for blood selenium level;the general clinical data of each patient were recorded in detail;the patient’s early morning fasting venous blood was collected within 24 hours after admission to test UA,ALT,AST,Hcy,Cre,NSE,TC,TG,LDL-C,HDL-C and other biochemical indicators.(3)SPSS 20.0 statistical software was used to analyze the relevant data.Results(1)Compared with the CAE patients in relatively low blood selenium group and relatively high blood selenium group,the blood selenium,ALT and AST levels were significantly different(P <0.05).It indicates that the liver function damage of CAE patients in relatively low blood selenium group is higher than that in relatively high blood selenium group.The two groups of CAE patients had no significant differences in age,smoking index,education years,drinking years,average daily alcohol consumption,UA,Hcy,Cre,NSE,TC,TG,LDL-C,and HDL-C levels(P > 0.05).(2)Compared with the CAE patients in relatively low blood selenium group and relatively high blood selenium group,the Evans’ index,Bicaudate index,Cella media index,Bifrontal index,and Huckman’s index were significantly different(P < 0.05).The difference in Ventricular index was not statistically significant(P > 0.05).(3)There were significant differences in Mo CA total score,executive function / visuospatial structure ability score,calculation power score and delayed recall score between the CAE patients in relatively low blood selenium group and the CAE patients in relatively high blood selenium group(P < 0.05).The differences in naming score,attention score,verbal fluency score,abstract score,and orientation score were not significant(P > 0.05).(4)Evans’ index was negatively correlated with Mo CA total score,executive function / visuospatial structure ability score,and calculation power score(P < 0.05).There were significant negative correlations between Ventricular index,Bifrontal index,and Huckman’s index with Mo CA total score,executive function /visuospatial structure ability score,calculation power score,and delayed recall score(P < 0.05).There were significant negative correlations between the Cella media index and Mo CA total score,executive function / visuospatial structure ability score,calculation power score,delayed recall score,and orientation score(P < 0.05).Evans’ index,Ventricular index,Bifrontal index and Huckman’s index did not correlate with naming score,attention score,verbal fluency score,abstraction score,or orientation score(P > 0.05).There was no correlation between Cella media index and naming score,attention score,verbal fluency score,or abstract score(P > 0.05).There was no correlation between the Ventricular index and Mo CA total score,executive function /visuospatial structure ability score,calculation power score,delayed recall score,naming score,attention score,verbal fluency score,abstract score,or orientation score(P > 0.05).(5)There was a negative correlation between blood selenium concentration and Evans’ index,Bicaudate index,Cella media index,Bifrontal index and Huckman’s index of brain atrophy index(P < 0.05).There was no correlation between blood selenium concentration and Ventricular index(P > 0.05).Blood selenium concentration was positively correlated with Mo CA total score,executive function / visuospatial structure score,computational power score and delayed recall score(P < 0.05).There was no correlation between the scores of naming,attention,language fluency,abstraction and orientation(P > 0.05).(6)AST and ALT were negatively correlated with blood selenium concentration(r =-0.195,-0.273,P <0.01).ALT was negatively correlated with the score of computational power in cognitive function(r =-0.260,P < 0.01).There was no correlation between ALT and cognitive function score,brain atrophy index(P > 0.05).AST had no correlation with brain atrophy index and cognitive function score(P > 0.05).Conclusion The blood selenium concentration of patients with CAE was negatively correlated with the degree of brain atrophy and ALT,AST in liver function;The blood selenium of patients with CAE was positively correlated with executive function,calculation ability and delayed memory ability;ALT level in liver function of patients with CAE was negatively correlated with calculation ability.
