| Objective:To explore the clinical efficacy of Guizhi Fuling Pills in the treatment of Calf muscle venous thrombosis(CMVT)after total knee arthroplasty(TKA),so as to provide a clinically better treatment for CMVT comprehensive therapy.Methods:Select 108 patients who meet the inclusion criteria from January 2020 to January 2021 and randomly divide them into three groups: control group 1(group A),control group 2(group B)and experimental group(group C),each with 36 groups example.Group A: Aspirin was administered orally within 24 hours after surgery,once a day,1 tablet once a time,for a period of≥1 month.Group B: Oral aspirin within 24 hours after the operation,the usage and dosage were the same as the control group 1.On the second day after the operation,the oral administration of rivaroxaban was started,once a day,2 tablets once a day,and the administration time was≥1 month.Group C: Start taking aspirin within 24 hours after operation,the usage and dosage are the same as control group 1.On the second day after operation,add Guizhi Fuling Pills Jiawei Granules,1 dose a day,2 times in the morning and evening,taking time≥1 month.Observation indicators: color Doppler flow imaging(CDFI)of lower limbs,D-dimer(DD),fibrinogen(Fbg),visual analogue scale(VAS),hospital for special surgery knee score(HSS)and TCM syndrome score.Results:The number of final follow-ups completed in control group 1(group A),control group 2(group B),and experimental group(group C)were 34,33,and 30,respectively.The baseline data of the three groups had no statistically significant differences in gender,age,height,weight,and surgical side(P>0.05),and they were comparable.1.CDFI results: At half a month after the operation,the number of people cured of thrombus in groups B and C was more than that in group A,the difference was statistically significant(P<0.05),and the difference between the remaining groups was not statistically significant.2.D-D value: At 5 days after the operation and half a month after the operation,the D-D value of the groups B and C was lower than that of the group A,the difference was statistically significant(P<0.05),and the difference between the other groups was not statistically significant.3.Fbg value: At 5 days after the operation and half a month after the operation,the Fbg value of the groups B and C was lower than that of the group A,the difference was statistically significant(P<0.05),and the difference between the other groups was not statistically significant.4.VAS score: At 5 days after surgery and half a month after surgery,the VAS scores of groups B and C were lower than those of group A,the difference was statistically significant(P<0.05),and the difference between the other groups was not statistically significant.5.HSS score: At 5 days after surgery and half a month after surgery,the HSS scores of groups B and C were higher than group A,the difference was statistically significant(P<0.05);at half a month after surgery,The HSS scores of group C were higher than that of group B,and the difference was statistically significant(P<0.05);the difference between the other groups was not statistically significant.6.TCM syndrome score and efficacy: After treatment,the TCM syndrome scores of group B and C were lower than group A,and the total effective rate was higher than group A,the difference was statistically significant(P<0.05).7.Safety observation: The vital signs of patients in the three groups were basically normal.In group A,1 case had tape allergy,2 cases in group B had subcutaneous ecchymosis,and 1 case in group C had subcutaneous ecchymosis,2 cases of constipation,and dry mouth.After symptomatic treatment,the symptoms improved significantly,none of which was discontinued.There was no significant difference in complications between the two groups(P>0.05).No serious complications such as suspected PE symptoms,proximal DVT and major bleeding events occurred.Conclusion:For patients with CMVT after TKA,the use of Guizhi Fuling Pills combined with aspirin is safe and effective.It can promote the healing of thrombosis within half a month after surgery,effectively relieve pain,accelerate the recovery of knee joint function,and reduce TCM syndrome points.It also reduces the levels of D-D and Fbg,which has unique clinical advantages and is worthy of promotion and application. |
