| 【 PURPOSE 】 Because it is not clear whether local application of vancomycin powder can reduce periprosthetic infection,this study aims to explore the application effect of local application of vancomycin powder in the prevention of periprosthetic infection after total hip arthroplasty and its impact on the incidence of postoperative infection,so as to provide a theoretical basis for clinical treatment.【 METHODS】from October 2016 to may 2020,285 patients with total hip arthroplasty in the Department of Orthopaedic Surgery of the First Affiliated Hospital of Bengbu Medical College were selected and randomly divided into two groups.The control group received routine postoperative periprosthetic infection prevention,and the study group received local application of vancomycin powder to prevent periprosthetic infection.The general information such as age,gender,height and body mass index,preoperative complications,hormone use,preoperative albumin,preoperative hemoglobin,operation time,postoperative infection rate were compared between the two groups,and the clinical characteristics of patients with infection were analyzed.There was no significant difference in the general data between the two groups(P > 0.05).The operation time,postoperative hemoglobin,delayed healing of incision,periprosthetic infection and the frequency of lower extremity deep venous thrombosis were recorded and analyzed by spss23.0 statistical software.【 RESULTS】: there were no significant differences in operation time,postoperative hemoglobin and serum albumin between the two groups(P >0.05).There were 5 patients with periprosthetic infection in the experimental group and 3 patients with periprosthetic infection in the control group.There were no significant differences in periprosthetic infection between the two groups(P > 0.05).There were 3 patients with lower extremity deep venous thrombosis in the experimental group and 2patients with lower extremity deep venous thrombosis in the control group,There was no significant difference between the two groups(P > 0.05).There were 16 cases of delayed healing in the experimental group and 3cases in the control group.There was significant difference between the two groups(P < 0.05).【 CONCLUSION】 in the prevention of periprosthetic infection after total hip arthroplasty,local application of conventional vancomycin powder can reduce periprosthetic infection and increase the risk of aseptic infection of incision.Therefore,the author thinks that it is not worthy of clinical routine use. |
