Objective:To investigate the safety and surgical superiority of the self-restraint Non-inflatable Endoscopic System to maintain the operation space.As well as the therapeutic effect of the modified double-port gasless transaxillary endoscopic thyroidectomy by this equipment in thyroid tumors.Methods:100 patients with papillary thyroid carcinoma who underwent thyroidectomy in the tumor surgery department of our hospital from October 2018 to October 2020 were divided into an endoscopic group(n = 46)and an open group(n = 54)on the basis of the surgical methods.In the endoscopic group,22 cases underwent single-port surgery and 24 cases underwent double-port surgery.Both single-port and double-port groups adopted the self-restraint gasless cavity construction equipment to perform gasless transaxillary endoscopic thyroidectomy.while in the open group,the traditional open anterior neck thyroidectomy was performed.The general clinical data of the two groups were compared and analyzed,and the relevant clinical indicators during the perioperative period(operative time,the intraoperative blood loss,24-hour postoperative drainage,number of central lymph node dissection,incision length,postoperative complications)were compared between the two groups.The postoperative pain score on the first day and cosmetic effect score was compared and analyzed.Results:The operations of all patients were carried out smoothly,and there was no conversion and opening in the endoscope group,the effect of the operation was satisfactory.Compared with the open group,the operative time and 24-hour postoperative drainage in the endoscopy group were significantly higher than those in the open group(P <0.05),while the incision length in the endoscopy group was smaller than that in the open group,and the evaluation of postoperative satisfaction in the endoscopy group was higher than that in the open group(P <0.05).There was no significant difference in intraoperative blood loss and the number of central lymph node dissection(P >0.05),but there was no significant difference in the total incidence of postoperative complications between the endoscope group and the open group(P >0.05).In the endoscope group,22 cases of thyroid surgery were performed by single-port method and 24 cases by double-port method.The operative time and incision length of double-port method were significantly lower than those of single-port method(P <0.05),but there was no significant difference in intraoperative blood loss,24-hour postoperative drainage,number of central lymph node dissection,pain score on the first day after operation and NSS score of cosmetic effect(P >0.05).Conclusion:Through improvement and optimization,the whole equipment of instruments has a good effect on maintaining the operation space,strong instrument support force and high spatial stability,and improves the safety of surgery.It was found in clinical application,double-port gasless transaxillary endoscopic thyroidectomy can shorten the operative time and reduce the length of incision;small incision is more conducive to postoperative recovery and improve cosmetic effect,and is more suitable for clinical promotion.
