| Objective: In this study,through the statistical analysis of minimally invasive transforaminal fusion(MIS-TLIF)and open posterior lumbar fusion(PLIF)in the treatment of single-segment lumbar degenerative diseases,the surgical effects,postoperative rehabilitation and long-term life effects of the two methods on patients with lumbar degenerative diseases were compared,and the advantages and disadvantages of the two methods were further analyzed.To provide a more sufficient basis for the selection of surgery for lumbar degenerative diseases in the later stage.Methods: A retrospective comparative study was conducted to collect 91 patients with single segmental lumbar degenerative diseases admitted in our hospital from September 2016 to September 2019.According to the different surgical methods,they were divided into two groups(MIS-TLIF group,n= 39 and PLIF group,n = 52).MIS-TLIF group was treated with transforminal lumbar interbody fusion under Quardant expandable channel and open posterior lumbar interbody fusion was performed in PLIF group.The postoperative follow-up period was more than 1 year.The clinical data,operation time,intraoperative blood loss,postoperative drainage,hospital stay,incidence of complications and interbody fusion rate were recorded and collected between the two groups.And waist and leg pain score and quality of life score at 1 week,3months,6 months and 12 months after operation were also collected.The pain score was evaluated by Visual Analogue Score(VAS),and the quality of life score was evaluated by Oswestry Dysfunction Index questionnaire(ODI).Intervertebral fusion was evaluated by the last lumbar X-ray film.All the above data are analyzed by statistics.Result:The operation time of MIS-TLIF group was longer than that of PLIF group,and the intraoperative blood loss,postoperative drainage and hospital stay were lower than those of PLIF group(P< 0.05).There was no significant difference in the incidence of complications and interbody fusion rate.The VAS score of the low back pain in MIS-TLIF group was better than that in PLIF group at 1 week and 3 months after operation.There was no significant difference in low back pain VAS score and leg pain VAS score at 6 and 12 months after operation.There was no significant difference in ODI score between the two groups after operation.Conclusion:Both MIS-TLIF and PLIF have good clinical effects on lumbar degenerative diseases,but MIS-TLIF has obvious advantages in intraoperative blood loss,postoperative drainage,hospital stay and early postoperative rehabilitation,which is worthy of clinical promotion.However,the long time and steep learning curve of MIS-TLIF surgery requires surgeons to make adequate preoperative preparation,grasp the surgical indications and accumulate rich surgical experience,so as to provide patients with a more reasonable treatment plan. |
