Objective: To evaluate the diagnostic value of different biological markers in patients with CRS4.Methods: A total of 415 patients with stage 5 chronic kidney disease were enrolled.Patients were divided into CRS4 group(208 cases)and CKD5group(207 cases)according to whether they had cardiac involvement,including cardiac function decline,left ventricular hypertrophy or diastolic dysfunction.Biomarkers such as IL-6,TNF-α,NT-pro BNP,sCysC,s NGAL,u NGAL and u NAG were collected for statistical analysis.Results: The levels of sCysC,s NGAL,u NGAL and u NAG in CRS4 patients were significantly higher than those in CKD5 group(P <0.05),while the expression levels of IL-6,TNF-α and NT-pro BNP had no significant differences(P > 0.05).Multivariate logistic regression analysis showed that u NGAL,gender,serum cholesterol,admission systolic blood pressure,admission diastolic blood pressure,hemoglobin and blood uric acid were independent influencing factors for the occurrence of CRS4(P < 0.05).Based on receiver operating characteristic(ROC)curve,the area under curve(AUC)of u NGAL was 0.728,the sensitivity was75.6%,and the specificity was 65.4%(P < 0.001).The area under curve(AUC)of sCysC was 0.654,the sensitivity was 51.1%,and the specificity was 76.9%(P=0.009).Conclusion: Our results suggest that u NGAL,Gender,Serum Cholesterol,Admission Systolic Blood Pressure,Hemoglobin and Serum Uric Acid are independent influencing factors of CRS4.u NGAL and sCysC may have certain diagnostic value in the diagnosis of CRS4,and u NGAL has a higher diagnostic value than sCysC.
