BackgroundEsophageal cancer is a common digestive tract tumor in China,and Henan Province is a high incidence area of esophageal cancer in China.With the improvement of patients’ health awareness and the progress of medical science and technology,esophageal cancer can be found and treated in the early and middle stages,among which radical esophagectomy is the main and preferred treatment.Because of the particularity of its anatomic location,radical resection of esophageal cancer usually takes a long time,causes great trauma to the body,high incidence of postoperative complications,and even death of the patient.Studies have shown that dexamethasone,as a commonly used glucocorticoid anti-inflammatory drug in clinic,can effectively reduce postoperative complications,but there is no relevant research on the perioperative application of dexamethasone in radical esophagectomy.ObjectiveThis paper studies whether dexamethasone and dosage are used in the induction period of anesthesia in patients with esophageal cancer radical surgery,and the results and related indexes of stable extubation time,enhanced analgesia and prevention of cold war and nausea and vomiting during anesthesia recovery period are analyzed,and the necessity and method of applying glucocorticoids are evaluated,it can provide reference for patients with esophageal cancer who are undergoing radical resection of esophageal cancer.MethodNinety patients in Xinxiang Central Hospital were recruited to this study with aged at50 to 80 years old,weighing from 45 to 80 kg,ASA grade at II-III,operation time less than5 hours from September 2018 to September 2019 in our hospital.Patients with inactive peptic ulcer,severe hypertension,diabetes,liver and kidney dysfunction and cardiopulmonary diseases were excluded.They were randomly divided into group A,group B and group C.Patients in each group were given intravenous injection of 10 mg dexamethasone,5 mg dexamethasone and normal saline during anesthesia induction.Patients in each group were given intravenous injection of 10 mg dexamethasone,5 mg dexamethasone and normal saline when anesthesia induction.The patient’s age,gender,height,weight,ASA grade,operation time,volume of rehydration,urine volume,blood loss,extubation time,anesthesia recovery room time,application of analgesics,nausea and vomiting,shivering,etc.were recorded.The body temperature,heart rate and mean arterial pressure of the patients were recorded after 5 minutes of rest in the operating room and 5minutes of rest after anesthesia resuscitation.ResultThere were no significant differences in age,gender,height,weight,urine volume,fluid volume,heart rate and mean arterial pressure among the three groups(P > 0.05).Compared with group C,the stable extubation time,anesthesia recovery room time and the incidence of shivering,nausea,vomiting and pain in group A and group B were significantly shorter(P < 0.05).Compared with group B,the extubation time and anesthesia recovery room time of group A were shorter(P < 0.05).ConclusionDexamethasone pre injection can reduce the incidence of postoperative nausea,vomiting,shivering and pain,shorten the extubation time after radical resection of esophageal cancer,and the effect of 10 mg dexamethasone was better than that of 5 mg dexamethasone.
