As a common chronic disease,Fibromyalgia(FM)is widely present in different populations.However,there are many controversies about its pathogenesis and treatment plan.At present,there are two main treatment schemes: drug therapy and non-drug therapy,which are usually used in combination,but there are evidences that long-term use of neuroactive drugs may lead to cognitive impairment.With the in-depth study of motor abnormalities in patients with chronic pain,more and more evidences support the therapeutic effect of motor cortex regulation in chronic diseases such as FM.Motor cortex activation has analgesia effect to chronic pain,so for the treatment of patients with FM seek more non-drug schemes,such as making the appropriate exercise methods.This paper selected 38 FM patients and 12 healthy subjects as the research objects.Electroencephalography(EEG)and Functional Near Infrared Spectroscopy(f NIRS)were used to record changes of brain metabolism and neuro electrical activity during finger tapping and laser stimulation damage,and to explore the complex mechanism between exercise and pain.My primary work is as following:The difference of neural electrical activity changes between FM subjects and healthy subjects was analyzed by Laser evoked potentials(LEPS)feature analysis method.As a kind of special event-related potential(ERP)associated with laser pain stimulation events,LEPS is often used as a useful tool to evaluate pain pathways in experimental pain and nervous system diseases.And f NIRS was analyzed by Statistical Parametric Mapping(SPM)method based on General Linear Model(GLM).According to the activation map of statistical inference,we analyzed the differences of brain metabolic activity between the two groups under different conditions and the differences of brain metabolic activity between the same group under different experimental conditions.Transfer entropy(TE)is a time series causality analysis method based on information theory,which has the direction of information transmission.In this paper,directed brain network analysis based on Transfer entropy is used to explore the complex influence process between exercise and pain.Through LEPS,it was found that FM patients showed different neural responses to all laser stimulation compared with healthy controls.The f NIRS statistical inference activation results showed that there were significant metabolic differences between the patient group and the control group under laser stimulation and rapid finger tapping exercise.Repeated slow finger tapping and other inertial movements did not help to relieve pain,while rapid finger tapping exercise had a certain relieving effect on pain in FM patients.The effective connection(EC)brain network based on TE method shows that communication of primary somatosensory cortex and motor cortex was increased under the condition of the laser stimulate and rapid finger tap.In the patient group,there was an enhancement of the directional information flow between the primary motor cortex and the primary somatosensory cortex,so it indicates that primary motor cortex tissue function may be improved and the primary somatosensory cortex inhibition has been restored.These results support the effectiveness of moderate exercise in alleviating pain in FM patients.
