Investigation And Analysis Of Community Service Demand For Health Management Of Elderly Chronic Diseases

With the increase of age,the physical function of the elderly declines,the living habits and diet structure change,and chronic diseases become the main health risks.The etiology of chronic diseases has the characteristics of diversity,complexity,strong concealment at onset,long treatment time,stubborn illness and easy repetition.At present,diabetes among the chronic diseases of the elderly in China is characterized by the most extensive harm,high incidence,long cycle and many complications,Therefore,the research on health management of type 2 diabetes can be used as a typical study to change the cognition and behavior of the elderly,strengthen the health management of chronic diseases of the elderly,improve the quality of life of the elderly and promote social harmony.In order to explore the community-home medical care service with the participation of social organizations with professional knowledge and the personalized health management model of chronic diseases for the elderly,the researcher investigated 116 elderly patients with type 2 diabetes by questionnaire,which involved the patients’ cognition of diabetes causes,treatment methods,diet,exercise and other related knowledge,mental health status and health management service needs.Through data analysis,problems such as insufficient cognition of diabetes and its nursing,imbalance between supply and demand of health management were found,which mainly reflected the following problems Second,the health management form of chronic diseases is single;Third,the community-home medical care service resources are insufficient;Fourth,the lifestyle of the elderly needs to be improved;Fifth,the mental health status of patients is poor.To provide personalized health management services(nursing services)for the elderly on the basis of community-home medical care,to refine management content,to strengthen health guidance for chronic diseases,to guide social attention,to increase investment in health management,to integrate social resources,and to explore management service models,so as to help improve patients’ health management awareness and improve the health management level of chronic diseases for the elderly,which is of great significance to promote the health of the elderly and improve their quality of life.
