Objective To investigate the causes of thrombocytopenia in patients with systemic lupus erythematosus and to explore the mechanisms by which it may occur.Methods The patients in the test group were 20 patients with SLE who visited the Department of Rheumatology and Immunology of the First Affiliated Hospital of Anhui Medical University between October 2019 and March 2021,and the health control group was 20 healthy people in outpatient physical examinations during the same period.The platelet-rich plasma(PRP)was obtained by separating the whole blood from the upper layer with a low-speed centrifuge,and then the platelets precipitated in the PRP were separated by centrifugation,and the upper liquid layer was removed to make a certain concentration of platelet suspension to detect the relevant parameters of platelet apoptosis.The PS ectopy rate and intracellular Caspase-3 content of platelets were detected by flow cytometry via fluorescent staining with Annexin V-FITC and Green Nuc? Caspase-3 Substrate;the fluorescent probes JC-1 and Fluo-3AM and DCFh-DA were labeled,and then flow cytometry was applied to detect changes in mitochondrial membrane potential,intracellular calcium ion concentration and intracellular reactive oxygen species content;and the differences of each index between the two groups were compared separately.Results The incidence of platelet PS ectopy(P<0.05),Ca2+ concentration(P<0.01),and intracellular ROS content(P < 0.001)in the SLE group significantly decreased compared with normal controls,and the differences were statistically significant;however,there were no significant differences in intracellular mitochondrial membrane potential and Caspase-3 content(P > 0.05),and the differences were not statistically significant.Conclusions In SLE patients,platelet apoptosis is one of the causes of thrombocytopenia,and disturbance of auto ROS metabolism and elevated Ca2+ concentration may be the mechanism leading to platelet apoptosis.
