Objective:The endocrine and metabolic characteristics of PCOS patients in Adolescent and reproductive age were compared,and the diagnosis and treatment strategies in different age groups were formulated to provide reference for stratified long-term management.Methods:A total of 328 patients with PCOS who were admitted to the Department of Gynecology and Endocrinology of the First Hospital of Shanxi Medical University from January 2018 to December 2019 were reviewed and summarized.88 patients were divided into adolescent group,including 34 obese patients and 54 non-obese patients.Of the 240 cases in the reproductive age group,96 were obese and 144 were non-obese.The general conditions(menarche age,course of disease,Kaohsiung signs,menstrual pattern,BMI,blood pressure,waist circumference,waist-to-hip ratio),LH/FSH,T,blood lipids and insulin resistance were compared between the two groups.Results:The incidence of amenorrhea in adolescent group was significantly higher than that in reproductive age group(P < 0.05).The incidence of irregular menstruation,waist-to-hip ratio,systolic blood pressure,TC,LDL,FPG,abnormal detection rate of TG and FPG in adolescent group were significantly lower than those in reproductive age group(P<0.05).In adolescent group,the levels of TG,LDL and insulin resistance in obese subgroup were significantly higher than those in non-obese subgroup,while the levels of LH/FSH and HDL in obese subgroup were significantly lower than those in non-obese subgroup(P < 0.05).In the reproductive age group,the levels of T,TG,FPG,FINS,HOMA-IR,HDL,FPG and the abnormal rate of insulin resistance in the obese subgroup were significantly higher than those in the non-obese subgroup,while the levels of LH/FSH and HDL in the obese subgroup were significantly lower than those in the non-obese subgroup(P<0.05).The reproductive age of age,course of disease,TC,LDL,FPG,HOMA-IR and abnormal rate of TG in non-obese subgroup were significantly higher than those in adolescent,while the age of reproductive age,FPG level and abnormal rate of FPG in obese subgroup were significantly higher than those in adolescent group(P<0.05).Conclusion:1.The incidence of irregular menstruation is higher in PCOS patients of reproductive age,and nearly 2/3 of them have menstrual disorder in adolescence.The incidence of adolescent PCOS amenorrhea is higher.2.Adolescent and reproductive age PCOS patients have lipid metabolism disorders and insulin resistance,especially in reproductive age,which is obviously manifested by increased FPG and TG.Obese reproductive age PCOS patients are more likely to have decreased HDL.3.Adolescent PCOS obesity and metabolic disorders are relatively mild,the main treatment for abnormal uterine bleeding(AUB),and also includes lifestyle management(diet + exercise + weight management).4.PCOS in reproductive age mainly targets at AUB,insulin resistance and Kaohsiung,while lifestyle intervention is also carried out.
