Meta Analysis Of Predictive Factors Of Idiopathic Inflammatory Myopathy With Interstitial Lung Disease

Objective:This study evaluated and analyzed systematically the risk factors of idiopathic inflammatory myopathy(IIM)-associated interstitial lung disease(ILD),which could provide evidence for early clinic diagnosis.Methods:Correlative literatures of the risk factors of idiopathic inflammatory myopathy-associated interstitial lung disease were searched from the English databases such as Pubmed,Embase,Cochrane Library Chinese databases such as CNKI database,Wanfang database,VIP database and China Biology Medicine database.According to the inclusion and exclusion criteria,high-quality literatures of risk factors of IIM-ILD were screened.The retrieval time was from 2000 to 2020.Revman 5.4 software was used for data analysis,consistency testing and sensitivity analysis.The publication bias was analyzed by funnel plot and Begg rank correlation test by Stata 16.0 software.Results:In this study,19 articles were contained,the overall sample capacity was 1895 cases,including 888 cases in the case group(IIM-ILD group)and 1007 cases in the control group(non-IIM-ILD group).Meta-analysis was performed on 18 risk factors.The results showed that :(1)Eleven studies were included to discuss the relationship between age and IIM-ILD,and the results were statistically significant [SMD=0.29,95%CI(0.16,0.42),P < 0.0001];(2)Thirteen studies were included to discuss the relationship between gender and IIM-ILD,and the results were not statistically significant [combined OR=0.96,95%CI(0.76,1.23),P=0.76];(3)Thirteen articles were included to discuss the relationship between fever and IIM-ILD,and the results were statistically significant[combined OR=3.31,95%CI(2.56,4.29),P < 0.00001];(4)Seven studies were included to discuss the relationship between arthritis and IIM-ILD,and the results were statistically significant [combined OR=2.85,95%CI(1.53,5.34),P=0.001];(5)Seven studies were included to discuss the relationship between arthralgia and IIM-ILD,and the results were statistically significant [combined OR=4.28,95%CI(2.44,7.51),P <0.00001];(6)Twelve studies were included to discuss the relationship between dysphagia and IIM-ILD,and the results were not statistically significant [combined OR=1.07,95%CI(0.61,1.87),P=0.82];(7)Ten studies were included to discuss the relationship between Raynaud’s phenomenon and IIM-ILD,and the results were statistically significant [combined OR=2.29,95%CI(1.50,3.48),P=0.0001];(8)Eight studies were included to discuss the relationship between malignant tumors and IIM-ILD,and the results were statistically significant [combined OR=0.32,95%CI(0.14,0.73),P=0.007];(9)Nine studies were included to discuss the relationship between the Gottron’s sign and IIM-ILD,and the results were not statistically significant [combined OR=0.67,95%CI(0.34,1.30),P=0.23];(10)Ten studies were included to discuss the relevance between existent antinuclear antibody and IIM-ILD,and the results were not statistical significance [combined OR=0.81,95%CI(0.62,1.06),P=0.12];(11)Twelve articles were included to discuss the relationship between anti-Jo-1 antibody positivity and IIM-ILD,and the results were statistically noteworthy [combined OR=4.10,95%CI(2.59,6.50),P < 0.00001];(12)Three articles were included to discuss the relevance between existent anti-SSA antibody and IIM-ILD,and the results were statistically noteworthy[combined OR=3.91,95%CI(2.41,6.34),P < 0.00001];(13)Five studies were included to discuss the relationship between lactate dehydrogenase and IIM-ILD,and the results were not statistically significant [SMD=-0.04,95%CI(-0.58,0.52),P=0.90];(14)Three literatures were included to discuss the relationship between hydroxybutyrate dehydrogenase and IIM-ILD,and the results were statistically significant [SMD=-0.04,95%CI(-,0.59,-0.21),P < 0.0001];(15)Seven studies were included to discuss the relationship between creatine kinase and IIM-ILD,and the results were not statistically significant [SMD=0.01,95%CI(-0.53,0.54),P=0.99];(16)Two studies were included to discuss the relationship between creatine kinase isoenzyme and IIM-ILD,and the results were not statistically significant [SMD=-0.63,95%CI(-1.85,0.59),P=0.31];(17)Eight literatures were included to discuss the relevance between ESR and IIM-ILD,and the results were statistically noteworthy [SMD=0.56,95%CI(0.24,0.87),P=0.0006];(18)Four studies were included to discuss the relationship between C-reactive protein and IIM-ILD,and the results were statistically significant [SMD=0.29,95%CI(0.06,0.53),P=0.01].Conclusion:(1)Older age,fever,arthritis,arthralgia,Raynaud’s phenomenon,positive anti-Jo-1antibody,positive anti-SSA antibody,increased ESR and increased CRP are the risk factors for IIM-associated ILD;(2)Malignant tumor and elevated hydroxybutyrate dehydrogenase are protective factors for the occurrence of ILD in patients with IIM;(3)Gender,dysphagia,Gottron’s sign,positive anti-nuclear antibody,lactate dehydrogenase,creatine kinase and creatine kinase isoenzyme are not related to the occurrence of ILD in patients with IIM.
