| Objective:To observe the clinical efficacy of laparoscopic sleeve gastrectomy in the treatment of type 2 diabetes mellitus complicated with obesity.Methods:Retrospective analysis of a total of 34 patients who underwent laparoscopic sleeve gastrectomy(gastric decompression)in the general surgery department of Bethune Hospital in Shanxi,China,from April 2013 to December 2019.The clinical data of patients before surgery,1,3,6,12 months and 3 and 5 years after surgery were recorded in follow-up,including:(1)Patients’ body mass-related indexes: height,body mass,BMI,percentage of overweight weight reduction;(2)Patients’ blood glucose-related indexes: glycosylated hemoglobin,fasting blood glucose,fasting insulin,insulin resistance index,and diabetes medication.The changes of each index were compared before and after surgery.Results:(1)All 34 patients underwent surgery successfully,and no serious postoperative complications were observed.(2)Body mass and BMI were lower than those before surgery at 1,3,6,12 months and 3 and 5 years after surgery(P<0.05);body mass and BMI gradually decreased at 3,6and 12 months after surgery(P<0.05);patients’ body mass and BMI were basically stable at 12 months after surgery and were at the plateau stage at 3 years after surgery.(3)Hb A1 c,FPG,FINS,and HOMA-IR were lower than those before surgery at 1,3,6,and 12 months and at 3 and 5 years after surgery,and Hb A1 c,FPG,FINS,and HOMA-IR gradually decreased at 3,6,and 12 months after surgery.They were slightly higher at 3 and 5 years postoperatively than at 12 months postoperatively(P<0.05).(4)Efficacy evaluation: 27 cases(79.4%)had complete remission,4 cases(11.8%)had partial remission,2 cases(5.8%)had improvement,and 1 case(3%)had failure.Conclusion:Laparoscopic sleeve gastrectomy has a good long-term effect in the treatment of type 2 diabetes mellitus complicated with obesity,and the control of body mass and blood glucose related indexes is relatively stable. |
