Objective: To evaluate the clinical efficacy of acupoint catgut embedding in treating postoperative pain of mixed hemorrhoids.Method: 180 patients with mixed hemorrhoids who were treated by external dissection and internal ligation of mixed hemorrhoids were selected from2019.09-2020.12 to the Anorectal Clinicl,First Affiliated Hospital of Anhui University of Chinese Medicine.They were randomly divided into acupoint catgut embedding group(group A),conventional analgesia group(group B),and blank control group(group C),and blank control group 60 cases.The acupoint catgut embedding group received acupoint catgut embedding therapy,the conventional analgesic group received intravenous dezocine,and the blank control group received no special intervention.However,there were 1 patient and 2 patients in the conventional analgesia group and the blank control group,respectively,who were treated with shedding.Compare 3 groups of patients at 24 h,48h,3rd,5th,7th day and other multiple periods of pain degree,pain duration,urination and sleep status,additional analgesic drug usage,adverse reaction incidence and satisfaction degree,and determine the clinical efficacy.Result: Result:1.The three groups of patients’ occupation,number of incisions,gender,smoking status,age,BMI,operation duration,basic pain threshold,preoperative pain score,preoperative SAS,SDS score,preoperative sleep,and other baseline data were balanced and comparable(all P>0.05).2.Compare patients’ pain: At 24 h after operation,the maximum VAS score and pain duration score of the conventional analgesia group was better than the other two groups,and the acupoint catgut embedding group was better than the blank control group(both P<0.05);After 48 hours,the two pain-related indexes of the conventional analgesia group and the acupoint catgut embedding group were better than the blank control group(both P<0.05),but the advantages of acupoint catgut embedding and dezocine were incomparable.(P>0.05);On the 3rd,5th,and 7th,the two pain-related indexes in the acupoint catgut embedding group were lower(shorter)than the conventional analgesia group and the blank control group,and the conventional analgesia group was lower(shorter)than the blank control group(all P<0.05).3.Compare the urination of patients: Unfortunately,at 24 h after the operation,none of the three treatments showed their own advantages in improving urination methods(P>0.05);After 48 hours,the urination of the acupoint catgut embedding group was better than that of the conventional analgesia group and the blank control group(both P<0.05).There was no statistical difference between the conventional analgesia group and the blank control group(P>0.05);On the 3rd and 5th day after operation,the urination of the acupoint catgut embedding group was better than that of the conventional analgesia group and the blank control group(all P<0.05),and the conventional analgesia group was better than the blank control group(all P<0.05);On the 7th day after operation,the urination of the acupoint catgut embedding group and the conventional analgesia group was better than that of the blank control group(both P<0.05),and the acupoint catgut implantation group is not better than the conventional analgesia group(P>0.05).4.Comparison of patients’ sleep status: 24 hours after operation,the sleep condition of the conventional analgesia group was better than that of the other two groups(all P<0.05),and the dominance of the acupoint catgut embedding group and the blank control group could not be compared(P>0.05);At 48 h and 7th day after operation,the sleep condition of the acupoint catgut embedding group was better than that of the other two groups(all P<0.05).Similarly,the sleep improvement effect of the conventional analgesic group was not better than that of the blank control group(P>0.05);On the 3rd day after operation,the acupoint catgut embedding group and the conventional acupuncture analgesia group were better than the blank control group(both P<0.05),but the superiority of the first two groups was not obvious(P>0.05);On the 5th day after operation,the sleep condition of the acupoint catgut embedding group was better than that of the conventional analgesia group and the blank control group(both P<0.05),and the conventional analgesia group was better than the blank control group(P<0.05).5.Comparing the use of additional analgesics in patients: the number and dosage of postoperative analgesics in the acupoint catgut embedding group were less than the other two groups(all P<0.05),and the conventional analgesia group was less than the blank control group(P<0.05).6.Comparison of patient satisfaction: The satisfaction of the blank control group,conventional analgesia group,and acupoint catgut embedding group increased progressively,and the acupoint embedding group was significantly stronger than the other two groups(all P <0.05).7.Efficacy evaluation: At 24 h after operation,the distribution of curative effect and total effective rate of conventional analgesia group was better than the other two groups(all P<0.05),and the acupoint catgut embedding group was better than the blank control group(all P<0.05);After 48 hours,the curative effect distribution and total effective rate of the conventional analgesia group and acupoint catgut embedding group were better than those of the blank control group.The advantages of intravenous drip and catgut embedding at acupoint cannot be compared(P>0.05);On the 3rd and 7th day after the operation,the overall curative effect of the acupoint catgut embedding group,conventional analgesia group,and blank control group decreased successively(all P<0.05);On the 5th day after the operation,the curative effect distribution of the acupoint catgut embedding group was better than that of the conventional analgesia group and the blank control group(all P<0.05),and the conventional analgesia group was better than the blank control group(all P<0.05);The total effective rate of the acupoint catgut embedding group was higher than that of the blank control group(P<0.05),and there was no statistical difference compared with the conventional analgesia group(P>0.05).8.Safety evaluation: The incidence of adverse reactions was 8.3% in the acupoint catgut embedding group,27.1% in the conventional analgesia group,and 44.8% in the blank control group.After statistical comparison,there are differences between the three groups of patients(all P<0.05).As the data shows,the acupoint catgut embedding group has the highest safety,followed by the conventional analgesia group.Conclusion: Acupoint catgut embedding has a good curative effect on postoperative pain of mixed hemorrhoids,high safety,easy clinical operation,and advanced painless intervention.It fully embodies the essence of the TCM theory of preventing disease and is suitable for clinical promotion and application.However,the curative effect of acupoint embedding in relieving acute postoperative pain is slightly insufficient.It is possible to consider embedding therapy 24 hours before surgery or with other analgesic measures to cover the "empty window" of acupoint embedding analgesia.As far as possible,patients can get a slight or even painless experience after surgery.
