Clinical Significance Of Serum CMV-IgM In Idiopathic Pulmonary Fibrosis

Objective:To explore the clinical importance of serum cytomegalovirus antibody IgM(CMV-IgM)in idiopathic pulmonary fibrosis(IPF).Methods:A total of 86 patients with IPF admitted to the Affiliated Hospital of Chengde Medical College from June 2019 to November 2020 were included in the retrospective study,and the study subjects were divided into two groups based on the serum CMV-IgM levels of the patients after admission,including 21 CMV-IgM-positive patients(CMV-IgM-positive group)and 30 serum CMV-IgM(CMV-IgM-positive group)and 65 patients with negative serum CMV-IgM(CMV-IgM-negative group),and 30 healthy patients who were examined in our hospital at the same time were selected as normal controls.The C-reactive protein(CRP),D-dimer,lymphocyte count(L),monocyte count(M)and white blood cell count(WBC)of each group were compared,and lung function measurements and high resolution CT(HRCT)scores were also compared between the CMV-IgM positive and negative groups.Results:The rate of CMV-IgM positivity in IPF patients was higher than that the normal control group,and the difference was statistically significant(P<0.05).CRP,D-Dimer,white blood cell count and monocyte count in the CMV-IgM positive group were all higher than those in the CMV-IgM negative and the normal control group,and the difference was statistically significant(P<0.05);lymphocyte count in the CMV-IgM positive group was lower than that in the CMV-IgM negative and the normal control group,and the difference was statistically significant(P<0.05).The DLCO% pred in the CMV-IgM positive group was lower than that in the negative group;HRCT score in the CMV-IgM positive group was higher than that in the negative group,and the difference was statistically significant(all P< 0.05).Conclusion:This study showed that the rate of CMV-IgM positivity was higher in IPF patients than in normal controls,and that CRP,D-Dimer,white blood cell count,monocyte count and HRCT score were elevated in IPF patients in the CMV-IgM positive group,while lymphocyte count and DLCO%pred were decreased,and CMV infection may be closely associated with IPF.
