Control Of Public Hospital Costs Under DRGs Mode

For a long time,the empirical cost control of public hospitals has the problems of complicated cost items and unclear division of cost control objects.As the medical reform gradually enters the " Profundal Zone ",the DRGs model will soon cover all public hospitals.At the same time,the "DRGs-PPS " implemented by medical insurance has further intensified the cost control problem of public hospitals,forcing public hospitals to carry out cost control reform.On the other hand,the outbreak of the new crown epidemic in 2020 has accelerated the digital revolution in the medical industry,providing an information platform and technical support for the cost control of public hospitals,and also promoted the reform of cost control in public hospitals.As an internationally recognized standardion model,DRGs model breaks the traditional result-oriented empirical management.This paper constructs a DRG cost control model in it,proposing a quantitative process control method,and solves the problems of the lack of process control in the traditional mode and the unclear division of cost control objects.The cost control activities described in this article include the formulation of cost control objectives beforehand,the use of EWMA control charts for process control,and the comparative analysis of medical capabilities,medical efficiency and medical quality through the DRG indicator control system after the event,and controling the variation in the link is corrected in real time,thus forming a complete closed management loop,which can not only guarantee the medical effect but also realize cost control.In addition,in order to verify the cost control effect of the DRG cost control model,This paper selects Y hospital in this city as the research object,and applying the DRG cost control model to Y hospital cost control for example analysis.Firstly,by analyzing the current cost control status of Y hospital,the corresponding cost control objectives were formulated;secondly,various expenses and hospitalization days that affect the total cost of FQ13 group were selected as the observation objects of process control,and the EWMA control chart was used for single value and multiple group observations,processing control analysis of Y hospital;third,complete the after-event control of Y hospital through the DRG index control system;fourth,evaluating the operation effect of the DRG cost control model in Y hospital,and verifying validity and operability of the DRG cost control model.Finally,in the process of implementing DRG cost control,Y hospital may face problems and current deficiencies that need to be improved,and put forward safeguard measures.This paper selects Y hospital in this city as the research object,applying the DRG cost control model to the cost control of Y hospital for example analysis,and selects the various expenses that affect the total cost of the FQ13 group.As the observation object of process control,hospitalization days are used for singlevalue and multi-group observations using EWMA control charts.In the post-event control activities,the DRG index control system is used to perform hierarchical analysis on Y hospital to evaluate and analyze the DRG cost control model in Y hospital.Operational effect to verify the effectiveness and operability of the DRG cost control model.Finally,in the process of implementing DRG cost control,Y hospitals may face problems and current deficiencies that need to be improved,and putting forward safeguard measures.This paper draws the following conclusions through research:(1)The EWMA control chart can realize the process control of the DRG group;(2)The DRG indicator control system reflects the medical efficiency while also taking into account the medical quality,medical capability and medical safety;(3)The DRG cost control model under the DRGs mode can achieve refined cost control.
