| Background:Among sports injuries,ankle sprain has a relatively high probability of occurrence.According to the survey,more than 20,000 people in the United States have ankle sprain every day.If there is no timely and correct diagnosis and treatment,acute ankle injury will further develop into CAI.Even with early conservative treatment,due to the inability of some patients to effectively immobilize the affected foot during rehabilitation,they will gradually develop into CAI,the characteristic manifestation is mainly sprain and swelling of ankle joint repeatedly.CAI is the main factor of osteoarthritis of ankle joint.Of NF-Kb in knee oa synovial membrane expression has attracted some attention,but for the ankle osteoarthritis from expression lack the pertinence of the synovial membrane and articular synovial wrapped to form a closed cavity,degenerative changes of articular cartilage and synovial hyperplasia,whether inflammatory change whether related needs further study.At present,ankle arthrodesis or total ankle arthroplasty is a classic method for the treatment of ankle OA,but they also have problems of limited function,high cost,infection and so on.Therefore,from the perspective of molecular cell biology to study the development and change mechanism of ankle instability to ankle osteoarthritis,how to reduce the incidence of osteoporosis,so as to improve the quality of life of patients has become a research hotspot.Objective:To investigate the role of NF-κB in the development of CAI to ankle osteoarthritis by detecting and comparing the differential expression of NF-κ B protein in the synovial tissue of patients with ankle sports injury and knee sports injury,so as to lay the foundation for the next step to explore the pathogenesis of osteoarthritis and provide the basis for the early intervention of ankle osteoarthritis Theoretical basis.And through the study of preoperative and postoperative ankle function score in patients with CAI,to explore the significance of surgical treatment for patients with ankle instability.To investigate the relationship between the expression of NF-κ B and ankle function in patients with CAI.Methods: In the CAI group,20 patients with CAI of ankle joint needed arthroscopic surgery,and in the matched group,20 patients with ACL rupture of knee joint needed arthroscopic surgery,The expression of NF-κ B in synovial tissue was tested by immunohistochemistry.The differences between the experimental group and the matched group were compared.The AOFAS score and VAS score of the experimental group before the operation and 6 months after operation were evaluated.The expression of NF-κ B in synovium of all ankle joints was compared with that of the control group AOFAS score and VAS score were analyzed.Results: The synovial tissue sections were stained with he and evaluated by optical microscope.The results showed that the Mankin score of the experimental group was(10.831.33),which was higher than that of the matched group(6.881.06)(P < 0.01).There was significant difference in Mankin score of HE staining between the two groups(P < 0.01).The IOD value of NF-κ B protein expression in the experimental group was(11.863.15),which was significantly higher than(3.721.06)in the control group(P < 0.01).The difference between the CAI groupand the matched group was significant(P < 0.01).The AOFAS score of the experimental group after ankle surgery was(74.213.98),which was significantly higher than(51.785.15)before surgery(P <0.01).The VAS score of chronic ankle joint unconcerned in the experimental group was(0.80.12),which was significantly lower than(6.502.31)before operation(P <0.01)The correlation coefficient between the IOD value of p65 protein expression and the preoperative AOFAS score of the experimental group was-0.925,P < 0.05,which was statistically significant,and there was a linear correlation between them,which was negative correlation;the correlation coefficient between the IOD value of NF-κB p65 protein expression and the preoperative VAS score of the CAI group was 0.967,P < 0.05,which was significant,and there was a linear correlation between them,which was positive correlation.Conclusion:.The Mankin score of synovial tissue in the experimental group was significantly higher than that in the patients with ACL injury;2.NF-κ B was expressed in both knee and ankle sports injury,and the expression in ankle sports injury group was higher than that in knee sports injury group,indicating that NF-κ B has relation with the pathogenesis and progress of ankle osteoarthritis;3 NF-κ B can be used as a biochemical marker to judge the severity of ankle instability;4.After arthroscopic surgery,the AOFAS score and VAS score of patients one year after surgery were significantly improved,and the quality of life was significantly improved. |
