| Objective: To study the effects of different dosage combinations of midazolam,sufentanil and propofol on hemodynamics during induction of general anesthesia.Methods:A total of 56 patients without hypertension who underwent elective non-cardiac surgery in the Second Affiliated Hospital of Hebei Medical University from January 2020 to January 2021 were randomly divided into 8 groups,with 7 people in each group.Midazolam,sufentanil and propofol were taken as the three factors,and each factor had two levels.The 2×2×2 factorial design table was selected to obtain 8 groups of anesthesia induction programs.Taking heart rate,cardiac output,peripher al vascular resistance,mean arterial pressure and central venous pressure before and after anesthesia induction as observation indexes,the main ef fect,interactive effect and simple effect of these three drugs on patients’ hemodynamics were analyzed.Every patient recorded the changes of he modynamic indexes before and after anesthesia induction with Piliphs ul trasonic doppler diagnostic instrument,and the results were analyzed by variance of three-factor factorial design data.Results:1.Midazolam is the main effective drug among the three drugs(P< 0.05),and the degree of lowering average arterial pressure by Midazo lam is dose-dependent.2.Sufentanil is the main effective drug affecting heart rate(P < 0.05).Sufentanil and propofol have interactive effects on heart rate(P <0.05).The combined application of 0.6ug/kg Sufentanil and 1.0mg/kg pr opofol has the greatest decrease in heart rate,while the combined applic ation of 0.4ug/kg Sufentanil and 1.0mg/kg propofol has the least effect on heart rate.3.Midazolam is the main effective drug among the three drugs(P<0.05).Sufentanil and propofol,Midazolam and Sufentanil have interacti ve effects on peripheral vascular resistance(P < 0.05).The combination of 0.6ug/kg Sufentanil and 1.0mg/kg propofol and the combination of0.05mg/kg midazolam and 0.6ug/kg sufentanil had the least effect on pe ripheral vascular resistance.4.The combination of these three anesthetic inducing drugs with di fferent doses has inhibitory effect on cardiac output,but there is no sig nificant difference among the groups.Conclusions:Midazolam reduces the mean arterial pressure by reducing the perip heral vascular resistance,while sufentanil mainly affects the heart rate,but slightly affects the circulatory system.The synergistic effect of prop ofol and sufentanil will obviously reduce the peripheral vascular resistan ce,so the dose of propofol should be reduced.In this study,the combi nation of midazolam(0.05mg/kg),sufentanil(0.6ug/kg)and propofol(1.0mg/kg)has the least effect on the peripheral vascular resistance of pati ents,and can be recommended for anesthesia induction. |
