Effect Evaluation Of CO₂ Lattice Laser Combined With Scar Plastic Surgery In The Treatment Of Burn Hypertrophic Scar

ObjectiveThrough the clinical follow-up observation of patients with burn hypertrophic scar who received treatment,the recovery of scar and the effect of two treatment methods in patients with scar plastic surgery and combined CO₂lattice laser treatment were analyzed.The specific situation of scar improvement before and after treatment was clarified,and the clinical efficacy and therapeutic value of photoelectric technology combined with surgery in the treatment of burn scar were discussed.Methods From December 2018 to June 2020,a total of 61 patients with burn hypertrophic scars treated with scar plastic surgery in the Department of Burn Plastic Surgery,General Hospital of Ning Xia Medical University（31 patients underwent laser treatment after surgery）were retrospectively analyzed.By comparing the VSS score and VAS score of scars before and after treatment,the therapeutic effect of scar plastic surgery combined with CO lattice laser on burn hypertrophic scars was analyzed.Results The scars of all included patients improved after treatment.Among the 30patients in the simple operation group,the average VSS score before treatment was10.50±1.43,and the average after treatment was 6.47±1.73.The average VAS（pain）score before treatment was 4.27±0.93,and the average after treatment was 2.07±0.68.The average VAS（pruritus）score was 5.13±0.84 before treatment and 2.40±0.84 after treatment.Before treatment,the VSS score was 10.39±1.38,and after treatment,the VSS score was 4.61±1.13.The VAS（pain）score was 4.42±1.10 before treatment and 2.00±0.62 after treatment.The VAS（pruritus）score was 5.16±0.88 before treatment.VAS（pruritus）score after treatment was 1.55±0.91.The VSS and VAS values of all patients decreased after treatment,and no recurrence occurred 6 months after treatment.Compared with the score difference before and after surgical treatment,the difference before and after laser combined with surgical treatment decreased more significantly,and the treatment effect was better.The VAS（pruritus）score of patients after combined treatment was significantly reduced（P<0.01）.Conclusions CO₂ lattice laser combined with scar plastic surgery is effective in the treatment of burn hypertrophic scar.After surgical treatment,the patient’s scar was released from contracture,the scar volume was reduced,and the scar traction pain was relieved to improve the softness and thickness of the scar.The surgery combined with laser treatment not only effectively improved the scar contracture,reduced the volume and hardness,but also significantly improved the color,vascular distribution,softness and thickness of the scar.Therefore,on the basis of scar plastic surgery combined with CO₂lattice laser in the treatment of burn hypertrophic scar can achieve better curative effect and clinical outcome.
