| Objective: To systematically evaluate the short-term clinical efficacy and safety of endoscopic surgery and conventional breast-conserving surgery in the treatment of early-stage breast cancer,a Meta-analysis of published clinical studies was conducted.Methods: The English databases included Cochrane Library,Pub Med,Embase and Web of Science,and the C hinese databases included C hina National K nowledge Infrastructure,Chinese Bio Medical Literature Database,China Science and Technology Journal Database and Wanfang Database.The search time of English literature is limited from July 2000 to December 2020,and the search time of C hinese literature is limited from July 2010 to December 2020,and the languages are limited to C hinese and English.A total of 27 studies involving 3,598 patients.Stata v.15.0 and Review Manager 5.3 software were used for data processing and statistical analysis.Outcome measures included: Prcedure time,Blood loss,Drainage time,Drainage volume,Lymph node harvesting quantity,Average duration of hospital stay,Length of the incision,Beauty effect satisfaction,Arm morbidities,Skin flap necrosis,incision infections and Subcutaneous hydrops.Results: There are 27 Chinese and English articles were included,including 23 retrospective studies,3 prospective studies,and 1 randomized controlled trial.,involving a total of 3598 patients with early-stage(stage I/II)breast cancer.There were 1810 cases in the conventional breast-conserving surgery group and 1788 cases in the endoscopic group.Meta-analysis results showed that there were no significant differences in Operation time,Drainage duration,Drainage volume and Lymph node harvested between the two groups(p>0.05).The Blood loss and Average duration of hospital stay in the endoscopic group were significantly less than those in the conventional breast-conserving group(p<0.05).The Arm morbidities,Skin flap necrosis,incision infections and Subcutaneous hydrops in endoscopic group were lower than those in conventional group(p<0.05).The Length of the incision and Beauty effect satisfaction of endoscopic group were significantly better than that of conventional breast-conserving group(p<0.05).Conclusion: Compared with traditional breast-conserving surgery.Endoscopic therapy in the cure of early breast cancer has less trauma,faster postoperative recovery,fewer complications and better satisfaction with cosmetic effect,which can provide certain theoretical support for future clinical work. |
