Correlation Between Dorsalis Pedis And Radial Arterial Invasive Blood Pressures During General Anesthesia Induction In Patients Undergoing Neurosurgery

Objective To investigate the correlation and consistency between dorsalis pedis and radial arterial invasive blood pressures during general anesthesia induction in patients undergoing neurosurgery,and to obtain the regression equation,so as to provide reference for clinical application of dorsalis pedis artery invasive arterial blood pressure monitoring.Methods Twenty patients who underwent neurosurgery during general anesthesia were enrolled in this study.The invasive arterial blood pressure of radial artery and dorsalis pedis artery was monitored in all patients after admission.Collect blood pressure data from 5 min before tracheal intubation to 5 min after tracheal intubation,and collect once every 10 s.Radial artery blood pressure(RA-BP)and dorsal artery blood pressure(DPA-BP)collected at the same time are paired data.After the data collection is completed,select the RA-SBP range of 90-139 mm Hg blood pressure data for analysis.The correlation and consistency between radial artery blood pressure and dorsalis pedis artery blood pressure in each group were analyzed,and the regression equation was obtained.Results(1)the correlation analysis between radial artery blood pressure and dorsalis pedis artery blood pressure and the establishment of linear regression equation.When the RA-SBP in the range of 90-139 mm Hg,there was a significant positive correlation between SBP,DBP and MAP of DPA and SBP,DBP and MAP of RA(The correlation coefficients of SBP,DBP and MAP were r=0.93,r=0.95,r=0.96,P<0.01).And the correlation coefficient r of MAP and DBP is greater than the correlation coefficient r of SBP(P<0.01);When the RA-SBP in the range of 90-139 mm Hg,the DPA-SBP was significantly higher than RA-SBP,and the DBP and MAP of DPA were lower than those of DBP and MAP in radial artery(P<0.01),but the difference was less than that of 5mm Hg.By univariate linear regression analysis.The formulas were obtained for the SBP,DBP and MAP of DPA and RA.When the RA-SBP is in the range of 90-139 mm Hg,SBP-dpa(mm Hg)=1.213×SBP-ra(mm Hg)-6.103,DBP-dpa(mm Hg)=0.866×DBP-ra(mm Hg)+4.953,and MAP-dpa(mm Hg)=0.957×MAP-ra(mm Hg)+ 2.049.(2)Analysis of consistency between radial artery blood pressure and dorsalis pedis artery blood pressure.When the RA-SBP in the range of 90-139 mm Hg,the results showed that the mean bias of SBP between RA and DPA was-18.6 mm Hg(-31.3~-5.9)mm Hg,the mean bias of DBP between RA and DPA was 3.3mm Hg(-2.6~9.2)mm Hg,the mean bias of MAP between RA and DPA was 1.5mm Hg(-3.9~6.9)mm Hg.the mean bias of SBP between DPA and RA exceeded the acceptable range,while the mean bias of DBP and MAP between DPA and RA were within the acceptable range.Conclusion There was a significant correlation between DPA and RA invasive arterial blood pressure when the RA-SBP in the range of 90-139 mm Hg.During the induction of anesthesia in neurosurgery,invasive blood pressure monitoring of DPA is as important as that of RA,invasive blood pressure monitoring of the DPA can replace RA blood pressure monitoring.And MAP might have more clinical reference significance than SBP when DPA is used for invasive blood pressure monitoring.
