| Objective: 1.Understand the status of life quality and depression of people living with HIV/AIDS.2.Exploring the possible influencing factors of the quality of life and depression of people living with HIV/AIDS.3.Exploring the relationship between depression and quality of life of people living with HIV/AIDS,and providing theoretical basis for the promotion of their mental health,the improvement of quality of life,and the formulation of AIDS prevention and treatment strategies.Methods: In this study,convenient sampling method was used to conduct a self-administered questionnaire survey on HIV / AIDS patients who were followed up and registered in CDC and infectious disease hospital.The questionnaire included general characteristics data,SF-36 scale and CES-D scale.The general demographic characteristics,HIV infection and sexual behavior,quality of life and depression were evaluated respectively.Frequency and constituent ratio were used to describe demographic characteristics,HIV related infection and sexual behavior;t-test,ANOVA and chi square test were used to compare the scores of quality of life and depression among target groups with different characteristics in univariate analysis,and multivariate linear regression and multivariate logistic regression were used for multivariate analysis.Pearson correlation was used to analyze the relationship between quality of life and depression,and structural equation model was constructed to verify the relationship among various influencing factors,depression and quality of life,and its effect intensity.Multiple quantile regression was used to explore the influence degree of depression on different quality of life.Results:1.In this survey,a total of 1051 questionnaires were sent out and 737 valid questionnaires were collected,with an effective rate of 70.12%.The highest score of each dimension of quality of life was physiological function(87.46 ± 16.64),and the lowest score was general health status(54.57 ± 19.05);the total score of physical health measurement based on the national norm standardized score was 47.42 ± 9.71,and the total score of mental health measurement was 44.63 ± 11.60,which were lower than the national model.The total score of depression was(21.65 ± 10.91),and the depression rate was 67.6%.2.The results of univariate analysis of quality of life showed that there were significant differences of the scores of physical health among people living with HIV/AIDS in different gender,age,marriage,family per capita monthly income,education level,occupation,route of infection,publicity of HIV infection,HIV viral load,CD4+T lymphocyte count,antiviral treatment,HIV testing and STD symptoms(P<0.05);There was statistical significance of the score of mental health of people living with HIV/AIDS in different occupation,income,education level,HIV viral load,route of infection and HIV infection disclosure(P<0.05).The results of multivariate analysis showed that: the presence of STD related symptoms,HIV viral load < 400(copies/ml),divorce or widowhood,receiving HIV testing,family per capita monthly income and CD4+T lymphocyte count were the influencing factors of physical health;the presence of STD related symptoms,HIV viral load ≥400(copies/ml),family per capita monthly income and receiving highly effective antiviral therapy were the influencing factors Influencing factors of mental health.3.The results of a single factor analysis of depression show that there are statistically significant differences in the depression rates of people living with HIV/AIDS among marital status,family per capita monthly income,occupation,education level,HIV viral load,and STD related symptoms.(P<0.05).The results of multivariate analysis showed that being married,farmers,low education level,low family monthly income,high HIV viral load and the appearance of STD symptoms are the influencing factors of depression.4.There was a negative correlation between the quality of life and depression among people living with HIV/AIDS(P<0.05);the results of structural equation modeling showed that depression had a direct negative effect on the quality of life,personal characteristics and HIV infection factors can have indirect effects on quality of life through depression.The influence degree of each factor on the quality of life was as follows: depression(-0.726),personal characteristics factor(0.298),HIV infection factor(0.177);the direct effects of personal characteristics factor and HIV infection factor on the quality of life were 0.153 and 0.094 respectively,and the indirect effects of depression on the quality of life were 0.145 and 0.083 respectively.The direct effect of personal characteristics factor on depression was-0.174,and the indirect effect of HIV infection factor on depression was-0.017.5.The results of multiple quantile regression showed that the regression coefficients of depression for all the 9 quantiles except P90 of physical health dimension were statistically significant(P < 0.001);the variation range of the regression coefficients of different quantiles of physical health dimension was-9.93~-2.61 and showed a gradual downward trend;the variation range of the regression coefficients of different quantiles of mental health dimension was-16.05~-11.11,The trend of P10~p75 was stable,but it increased sharply after p75.It showed that depression had no effect on HIV infected/AIDS patients with good physical health.The worse the physical health dimension was,the more serious the impact of depression symptoms was.The impact of depression on mental health was greater than that of physical health,and the impact on PLWHA with good mental health was greater.Conclusion:1.The quality of life of HIV / AIDS patients is low,and the main influencing factors are: marital status,family per capita monthly income,route of HIV infection,whether received highly effective anti-retroviral therapy,HIV viral load,CD4+cell count,whether received HIV testing and whether appeared STD related symptoms.2.Marital status,occupation,education level,per capita monthly income of family,HIV viral load and STD symptoms are the influencing factors of depression.3.The depressive status of HIV / AIDS patients has a negative effect on the quality of life.So we can take targeted measures to improve the quality of life by improving the depressive status. |
