Objective: Systemic lupus erythematosus(systemiclupuserythematosus,SLE)is a course of slowly diffuse connective tissue disease,its clinical harm is big,can cause a variety of tissues and organs and systems of injury,affect kidney and central nervous system symptoms heavier.The morbidity is high and the mortality rate of severe patients is high,which mainly harms women of childbearing age and seriously affects their survival and quality of life.Complement is commonly found in body fluids(such as serum and tissue fluid)and the surface of cell membranes,and is an important component of the immune system,among which complement C3 and C4 are more common.There are a large number and variety of autoantibodies in the serum of SLE patients.These antibodies combine with autoantigens to form a large number of immune complexes,thus activating the complement system and forming inflammatory damage.The changes of serum complement C3 and C4 levels in patients with SLE under different disease states were observed and their clinical significance was discussed.Methods: A total of 326 patients with a definite diagnosis of systemic lupus erythematosus(using the 1997 SLE classification recommended by the American College of Rheumatology(ACR),who met 4 or more of the 11 criteria and were diagnosed with SLE after the exclusion of infection,tumor,and other connective tissue diseases)were selected as subjects.(1)Ds DNA antibody was detected by indirect immunofluorescence method.According to the normal and abnormal anti-DS-DNA antibody results,they were divided into normal and abnormal anti-DS-DNA antibody groups.(2)ANA was detected by indirect immunofluorescence.According to ANA karyotypes,ANA was classified into homogeneous type,speckled type,nucleolar type and other types.Serum complement C3 and C4 contents were measured by scattering turbidimetry,and the changes of complement C3 and C4 in patients under different conditions were compared.Results: A total of 326 patients with a definite diagnosis of systemic lupus erythematosus were selected.There were 35 males and 291 females,ranging in age from 9 to 83 years old,with an average age of 40.2±15.5 years old.According to normal and abnormal results of anti-DS-DNA antibody,the patients were divided into normal group(238 cases)and abnormal group(85 cases).(1)The level of complement C3 in patients with abnormal anti-DS-DNA antibody was lower than that in patients with normal dsDNA,and the difference was statistically significant(P < 0.05).(2)The level of complement C4 in patients with abnormal anti-DS-DNA antibody was lower than that in patients with normal dsDNA,and the difference was statistically significant(P < 0.05).According to ANA karyotypes,there were 47 cases of homogeneous type,69 cases of speckled type,20 cases of nucleolus type and 152 cases of other type.(1)For complement C3,F = 2.656 and P < 0.05 were calculated by the method of one-way ANOVA.There were statistically significant differences in the overall mean of ANA karyotypes,indicating that the decrease of complement C3 levels in ANA karyotypes was not all the same.Postmortinal comparison(pair comparison)among each group showed that there was no statistically significant difference among homogeneous type,other type and speckled type,but P < 0.05 when homogeneous type and other type were compared with nucleolar type,so the differences between nucleolar type,homogeneous type and other type were statistically significant.As can be seen from the bar chart and broken line chart,the level of complement C3 in serum of SLE patients with various ANA karyotypes has a general trend of decline.(2)For complement C4,F = 2.702 and P <0.05 were calculated by the method of one-way analysis of variance.There was a statistically significant difference in the overall mean of ANA karyotypes,indicating that the decrease of complement C4 levels in ANA karyotypes was not all the same.After the postmortinal comparison(pairwise comparison)between each group,it was found that compared with the nucleolar group,there were P < 0.05,so the differences among nucleolar type,homogeneous type,other type and speckled type were statistically significant.As can be seen from the bar chart and broken line chart,the level of complement C4 in serum of SLE patients with various ANA karyotypes has a general trend of decline.Conclusion: The levels of complement C3 and C4 are different in patients with SLE under different disease states,and the combined detection of complement C3 and C4 is helpful to determine the disease activity of SLE and the types of autoantibodies.
