Objective:At present the incidence of lung cancer is more and more toward younger,but few studies of young patients with pulmonary nodules group,young patients of pulmonary nodules misdiagnosis rate is high,this study based on the analysis of young patients with pulmonary nodules group general clinical characteristics,imaging findings and tumor markers,etc,to establish the forecast model of predicting benign and malignant nodules,for the clinical practice of young patients with pulmonary nodules of benign and malignant diagnosis assistance and advice.Method:Selection in January 2016-December 2020,the first hospital affiliated to China medical university line thoracic surgical treatment of lung age less than the age of 45patients,to collect the patient’s gender,smoking history,family history,clinical symptoms such as cough and expectoration,chest distress,shortness of breath,chest pain,hemoptysis,low thermal fatigue and the nodal diameter,solid nodules composition,imaging findings such as burr sign,pleural pull sign,marginal incompleteness,vacuole sign,lobulation sign and vascular cluster sign,tumor markers and postoperative pathological information,etc.Patients with a total of 525 cases,of which during January2018 to December 2020,a total of 394 cases of patients with pathologic result divided into benign and malignant groups,for the statistical analysis of the above information will adopt spss23.0 binary Logistic regression analysis and take more factors,will filter out the independent risk factors of nodules of benign and malignant regression equation model is used to establish prediction,in January 2016-December 2017 a total of 131cases are used to verify effectiveness model validation data.Results:This study of 394 cases of young patients,malignant nodules in 223 cases,171cases of benign nodules in the two groups in gender,nodal maximum diameter,nodules solid elements,cluster sign,empty bubble sign,pleural retraction of the indications,differences was statistically and nodal independent risk factors for malignant,established nodules benign and malignancy risk prediction model for Y=e~x/1+e~x,X=0.539+(0.874×sex)+(0.163×maximum diameter of nodule)+(0.620×solid component)+(1.017×vascular cluster sign)+(0.531×pleural pulling sign)+(0.461×lobulation sign)+(0.465×vacuole sign),E=2.718281828.The validation data were substituted in,and the ROC curve was plotted according to the predicted probability.The area under the curve was 0.873,95%CI:0.805-0.941.When the cutoff value was 0.556,the model had the best sensitivity and specificity for predicting benign and malignant nodules,which were 93.4%and 72.7%,respectively.Conclusion:Gender,pulmonary nodule diameter size,whether has the essence ingredient,whether to have the pleura pull indications,lobulation sign,blood vessels,and cavitation character is a young group nodules of benign and malignant lung nodules patients important influence factors,this research through the above information to establish a benign and malignant nodules diagnostic performance risk prediction model is higher,can assist the clinical diagnosis of young patients with pulmonary nodules.
