ObjectiveTo analyze the related factors of hemorrhage corpus luteum cyst and adnexal torsion in differential diagnosis.MethodsThe clinical data of 40 cases of hemorrhage corpus luteum cyst and 31 cases of adnexal torsion were collected in Affiliated Hospital of Tai’an Medical College from December 2012 to October 2018 for retrospective research.Including the age of the patient,motherhood,history of lower abdominal surgery,causes of disease(strenuous exercise,sex life),time of onset from last menstrual period,accompanying symptoms(nausea and vomiting,vaginal bleeding,feeling of anal collapse,postural pain),search(peritoneal irritation,cervical pain),laboratory examination(hemoglobin,red blood cell count,white blood cell count,neutrophils ratio,CA125,CA199,HE4),ultrasonic(location,mass composition(cystic,cystic and solid),pelvic effusion,resistivity index,peak systolic velocity),MRI(maximum diameter of mass,whether the border is clear,there is no separation within the mass,T1WI signal,T2WI signal,T2WI signal)and so on.Conduct t test on measurement data,counting data were tested by chi-square test or Fisher exact test,single factor analysis was statistically significant project in multiariable logistic regression analysis.The hazard ratio(HR)and 95% CI of hemorrhage corpus luteum cyst were calculated.All statistical tests with P<0.05 were considered statistically significant.Results1.The age of the patient,causes of disease,feeling of anal collapse,ultrasound(pelvic effusion,mass composition(cystic,cystic solid),resistivity index,peak systolic velocity),MRI findings(maximum diameter,whether the border is clear),the difference was statistically significant(P<0.05).The motherhood,history of lower abdominal surgery,time of onset from last menstrual period,vaginal bleeding,postural pain,hemoglobin,red blood cell count,white blood cell count,neutrphils ratio,CA125,CA199,HE4,there is no separation within the mass,T1WI signal,T2WI signal,T2WI signal,the difference was not statistically significant(P >0.05).2.Multivariate analysis showed that he diameter of the mass was <5cm,unclear boundary,resistivity index <0.55,pelvic effusion,independent risk factors for hemorrhage corpus luteum cyst.3.Hemorrhage corpus luteum cyst was calculated by summing the number of risk factors.Higher hemorrhage corpus luteum cyst was associated with increased risk of hemorrhage corpus luteum cyst.Conclusion1.Hemorrhage corpus luteum cyst occurs most often in women of reproductive age,it has many triggers,mainly sex and vigorous activity.2.If more than two of the following four items meet the requirements,the more likely it is to be diagnosed as hemorrhage corpus luteum cyst: the diameter of the mass was <5cm,unclear boundary,low resistivity index <0.55,pelvic effusion.3.MRI can assist diagnosis when hemorrhage corpus luteum cyst and adnexal torsion are difficult to diagnose.
