Observation On The Therapeutic Effect Of Upper Limb Aerobic Training On Lung Rehabilitation In Patients With Complete Thoracic Spinal Cord Injury

Objective:To investigate the clinical effect of upper limb aerobic training on lung rehabilitation in patients with complete thoracic spinal cord injury.Methods:A total of 30 patients with thoracic spinal cord injury(T1-T12)treated in the Department of Emergency,Orthopaedics and Rehabilitation of the affiliated Hospital of Guizhou Medical University were randomly divided into routine rehabilitation group(n = 15)and aerobic training group(n = 15).Another 15 normal persons were selected and included in the normal control group.Patients in the routine rehabilitation group and aerobic training group were given routine rehabilitation training,including good posture,joint range of motion training,stretching training,muscle strength training,sitting balance training,activities of daily living training,psychotherapy and so on.5 days a week,2 hours a day for 4 weeks as a course of treatment.In the aerobic training group,the upper limb aerobic training was added on the basis of routine rehabilitation,and the exercise intensity was individualized according to the results of cardiopulmonary exercise test.The exercise frequency and time were 5 days a week and 30 minutes a day.4 weeks as a course of treatment.There was no intervention in the normal control group.Diaphragm function,pulmonary function,dyspnea index,ability of life,quality of life and pulmonary infection rate were assessed at the time of selection and 4 weeks after rehabilitation treatment.Results:All the 45 subjects completed the test successfully.Before treatment,there was no significant difference in general data,pulmonary function,diaphragm function,dyspnea index,activities of daily living and quality of life between the routine rehabilitation group and the aerobic training group(P > 0.05).Comparable.However,compared with the normal control group,the pulmonary function,diaphragm movement amplitude under maximum deep breath,activities of daily living and quality of life in the routine rehabilitation group and aerobic training group were significantly lower,and the dyspnea index was higher than that in the normal control group.the difference was statistically significant(P < 0.05).After 4 weeks of rehabilitation treatment,the pulmonary function,diaphragm function(except the amplitude of diaphragm movement under calm breathing),activities of daily living and quality of life in the routine rehabilitation group and aerobic training group were still lower than those in the normal control group.the dyspnea index was higher than that in the normal control group,but compared with that before treatment,the above indexes were improved in varying degrees,and the difference was statistically significant(P < 0.05).Although the pulmonary infection rate increased,the difference before and after treatment showed that the pulmonary infection rate in the aerobic training group was lower than that in the routine rehabilitation group.The above evaluation results showed that the indexes(except the amplitude of diaphragm movement under calm breathing)in the aerobic training group were significantly better than those in the routine rehabilitation group(P < 0.05).There was no significant difference in the amplitude of diaphragm movement under calm breathing before and after treatment(P > 0.05).Conclusion:Upper limb aerobic training can significantly improve lung function,activities of daily life and quality of life of patients with complete thoracic spinal cord injury.
