| Objective:To observe the therapeutic effect of lung rehabilitation exercise training combined with bronchodilator on pulmonary function,blood gas analysis,exercise tolerance,symptom score and risk of acute attack in patients with moderate and severe chronic obstructive pulmonary disease(COPD)in stable stage.Methods:According to the inclusion criteria and exclusion criteria,75 patients with moderate and severe stable COPD in the outpatient clinic of the First Hospital of Jilin University from January 2021 to November 2021 were selected for a 12-week clinical observation and study.75 patients were randomly divided into two groups: the control group received inhalation of bronchodilator(ANORO),while the observation group received lung rehabilitation exercise training in combination with inhalation of bronchodilator.Pulmonary function,arterial blood gas analysis,6-minute walking distance,the COPD assessment test(CAT)score,modified British Medical Research Council(m MRC)score and the number of acute attacks were evaluated in both groups.Results:1.Comparison of general data: there was no significant difference in sex,age,course of disease,routine white blood cell count and GLOD pulmonary function grade between the two groups(P > 0.05).2.Comparison of pulmonary function: there was no significant difference in FVC,FEV1%pred and FEV1/FVC between the two groups before treatment(P > 0.05).After 4 weeks of intervention,there was no significant change in the above-mentioned indexes in the two groups compared with those before treatment(P > 0.05).After 8 and 12 weeks of intervention,the above-mentioned indexes in the two groups were significantly improved as compared with those before treatment(P < 0.05),and the improvement in the observation group was more significant than that in the control group(P < 0.05).3.Comparison of arterial blood gas analysis: there was no significant difference in the values of Pa O2,Pa CO2 and Sa O2 in arterial blood gas between the two groups before treatment(P > 0.05).After 4 weeks of intervention,there was no significant change in the above-mentioned indexes in the two groups compared with those before treatment(P > 0.05).After 8 and 12 weeks of intervention,the above-mentioned indexes in the two groups were significantly improved as compared with those before treatment(P < 0.05),and the improvement in the observation group was more significant than that in the control group(P < 0.05).4.Comparison of 6-minute walking distance: there was no significant difference in 6-minute walking distance between the two groups before treatment(P > 0.05).After 4 weeks of intervention,there was no significant increase in the above-mentioned indexes in the two groups compared with those before treatment(P > 0.05).After 8 and 12 weeks of intervention,the above-mentioned indexes in the two groups were significantly higher than those before treatment(P < 0.05),and the increase in the observation group was more significant than that in the control group(P < 0.05).5.Comparison of symptom scores: there was no significant difference in CAT score and m MRC score between the two groups before treatment(P > 0.05).After 4 weeks of intervention treatment,the above-mentioned indexes in the two groups were not significantly lower than those before treatment(P > 0.05).After 8 and 12 weeks of intervention,the above-mentioned indexes in the two groups were significantly lower than those before treatment(P < 0.05),and the decrease in the observation group was more significant than that in the control group(P < 0.05).6.Comparison of the number of acute attacks: during the follow-up of 12 weeks,the number of acute attacks in the observation group was significantly lower than that in the control group,and the difference was statistically significant(P < 0.05).Conclusion:1.Inhaling bronchodilators can effectively relieve symptoms,improve lung function,exercise endurance and reduce the number of acute attacks in patients with moderate and severe COPD in stable stage.It is a routine treatment for patients with stable COPD.2.Compared with inhaling bronchodilator alone,combining it with lung rehabilitation exercise training is more effective in patients with moderate and severe COPD in stable stage,and can reduce the risk of acute attack,which is worthy of clinical popularization and application.3.Lung rehabilitation training is a simple,safe and effective exercise method,but its short-term effect is not good,so long-term training is needed. |
