| Background Chlamydia psittaci pneumonia is a zoonosis caused by ingestion of contaminated fecal matter,eyes and nasal secretions from infected birds and poultry through respiratory tract.In the past,due to the difficulties in pathogen culture,the delay of serology retrospective diagnosis,and the lack of PCR technology in hospitals, human psittacosis is ofen unconfirmed and inadequately diagnsed.With the widespread application of next-generation sequencing(NGS)technology in the etiological diagnosis,the number of chlamydia psittaci detected by NGS is gradually increasing.Combined with epidemiology,chlamydia psittaci can be clearly diagnosed as the pathogenic agent of CAP.More cases are available for clinical features summary.Part of similar clinical characteristics between chlamydia psittaci pneumonia and Mycoplasma pneumoniae pneumonia(MPP)have been found,the differences have not been compared.Therefore,in this study,the general situation,clinical manifestations,laboratory indicators and imaging manifestations of chlamydia psitsiti pneumonia group and MPP group were compared to explore the clinical similarities and differences of the two atypical pathogens pneumonia,and to strengthen a comprehensive understanding of Chlamydia psittaci pneumonia.Objective To collect clinical data of chlamydia psittaci pneumonia patients and MPP patients,retrospectively analyze their general data,clinical symptoms,laboratory examination indicators and imaging findings,summarize their clinical characteristics,and guide the clinical early diagnosis and antidiastole.Methods 24 patients with Chlamydia psittaci pneumonia admitted to the Second Affiliated Hospital of Anhui Medical University from January 2019 to January 2021 were selected as the case group,and 34 patients with pulmonary MPP during the same period were selected as the control group.The general situation,clinical symptoms,laboratory examination indicators and chest CT findings of the two groups were compared.Results The age,hospitalization days and peak fever of chlamydia psittaci pneumonia group were higher than MPP group.The proportion of first onset fever,chills and auscultation of wet rales was higher than MPP group,while the proportion of cough and stuffy nose was lower than MPP group.The levels of procalcitonin(PCT),interleukin-6(IL-6),C-reactive protein(CRP),neutrophil count(Neut),neutrophil percentage(Neut%),alanine aminotransferase(ALT),aspartate aminotransferase(AST)and direct bilirubin(DBi L)in chlamydia psittaci pneumonia group were higher than those in MPP group.Eosinophil count(Eos)and Basophilic granulocyte count(Baso)were lower than MPP group,and the differences were statistically significant(P < 0.05).Compared with the MPP group,the imaging manifestations of chlamydia psittaci pneumonia group showed consolidation shadow,bronchial inflation sign,pleural effusion and pericardial effusion,but less left lower lobe involvement,ground glass shadow,bronchial wall thickening,interstitial thickening and tree bud sign,and the differences were statistically significant(P < 0.05).Conclusions The onset age,peak temperature,hospital stay of Chlamydia psittaci pneumonia patients are higher than that of MPP.Inflammatory biomarkers,transaminase,direct bilirubin level are higher than that of MPP.Psittacosis have lower Eos and Easo account.Imaging performance show consolidation shadows,bronchial inflation sign,pleural effusion and pericardial effusion,less performance for lower left lung involvement.The above clinical manifestations,laboratory examination indicators and imaging findings provide reliable basis for early differential diagnosis of chlamydia psittaci pneumonia and MPP. |
