Objective To observe the effects of ultrasound-guided continuous supra-inguinal fascia iliac compartment block(FICB)on postoperative pain and early movement in patients undergoing unilateral total knee arthroplasty(TKA).Methods 80 patients who underwent elective TKA for the first time in our hospital from May 2021 to October 2021 were selected and randomly divided into patientcontrolled intravenous analgesia(PCIA)(group PCIA)and continuous suprainguinal fascia iliac compartment block(FICB)(group FICB).Group PCIA was connected with intravenous analgesia pump(sufentanil 2μg/kg+ droperidol 5mg+ saline diluted to 100ml)after operation.In group FICB,fascia iliac compartment was inserted the catheter on the groin under ultrasound guidance on the operative side before anesthesia,and the analgesic pump(300ml 0.2%ropivacaine)was connected postoperatively.Both groups were treated with an orthopedic "cocktail".Clinical indicators,NRS scores,Bromage scales and adverse reactions were compared between two groups.Results There were no significant differences in duration of surgery,anesthesia,tourniquet,PACU stays and intraoperative propofol dosage between the two groups(P > 0.05).Compared with group PCIA,intraoperative remifentanil,number of analgesic pump presses,tramadol dosage,first time to get out of bed,postoperative hospital stays in group FICB were remarkably lower(P <0.05).NRS score in resting at 6h and 24 h after surgery and in 5-meter walk test at 24 h and 48 h after surgery were remarkably lower(P <0.05).Bromage scales in 24 h and 48 h postoperatively were no notably differences between two groups(P > 0.05).Conclusion Continuous supra-inguinal FICB guided by ultrasound plays a good role in perioperative analgesia in patients undergoing total knee arthroplasty.Compared with PCIA,FICB can promote patients to get out of bed earlier,reduce perioperative opioid dosage,accelerate patients’ recovery and discharge from hospital,and without significant analgesia-related adverse reactions.
