| Objective This article aims to conduct nutritional assessment in patients with chronic obstructive pulmonary disease through a simple and effective assessment method to understand the nutritional status of hospitalized patients,analyze the risk factors of chronic obstructive pulmonary disease complicated by malnutrition,and the impact of Investigate malnutrition The impact of malnutrition The impact of malnutrition on the condition and quality of life of COPD patients provides a basis for future clinical nutritional interventions to reduce patient hospital stay,reduce economic burden and improve survival time.Methods Between September 2020 and February 2021,our hospital received 110elderly COPD patients who volunteered for the study and signed an informed consent form the following clinical data were collected from patients with chronic obstructive pulmonary disease,including: 1.Questionnaire survey: nutritional assessment of patients by MNA-SF,grading of patients’ current dyspnea by CAT and m MRC,and comprehensive evaluation of chronic obstructive pulmonary disease.Results classified patients into four levels of ABCD;2.General basic information: gender,height,age,drinking history,smoking history,ventilator use,risk of acute exacerbation within one year,duration of illness,type of drug used,surgery,lung function class,comorbid diseases(hypertension,coronary atherosclerosis)sclerosis,diabetes,pulmonary encephalopathy,bullae,acute myocardial infarction,cerebral infarction,respiratory failure,cardiac insufficiency,etc.)3.Biochemical data: total white blood cell count,red blood cell count,lymphocyte ratio,hemoglobin,albumin,total protein,creatinine,C-reactive protein,procalcitonin,triglyceride,total cholesterol,etc.According to the MNA-SF nutritional assessment criteria,they were divided into a nutritionally normal group and a nutritionally abnormal group.On the basis of the single-variable analysis of these indicators,the risk factors of malnutrition in elderly patients with COPD were investigated by logistic regression analysis.Results Among the 110 elderly COPD patients in this study,73 were in the normal nutrition group(66.4%),61 were male(83.6%),12 were female(16.4%),and 37 were in the abnormal nutrition group(33.6%),26 males(70.3%)and 11 females(29.7%).There were statistically significant differences between normal nutrition group and abnormal nutrition group in the following factors(P<0.05): age,BMI,CAT score,m MRC grade,alcohol consumption,risk of acute exacerbation within one year,duration of illness,use or not Ventilator,type of comorbidities.Taking nutrition status as the dependent variable.Logistic multifactor regression analysis showed that age,risk of acute exacerbation within one year,duration of illness,types of comorbidities,and whether or not to use a ventilator were all less than 0.05.The difference was statistically significant,and it can be considered that these five factors are the independent influencing factors of chronic obstructive pulmonary disease complicated with nutritional problems.Conclusion 1.The nutritional status of elderly COPD patients is related to factors such as age,risk of acute exacerbation within one year,duration of illness,type of co-morbidities,and use of ventilator2.The lower the MNA-SF score of elderly COPD patients,the higher the CAT score,m MRC classification and comprehensive evaluation. |
