Objective Diabetes mellitus and thyroid related diseases both belong to endocrine immune metabolic diseases.Recent studies have shown that elderly patients with type 2 diabetes tend to be complicated with thyroid related diseases,among which subclinical hypothyroidism and thyroid antibody abnormalities are the most common.It is of great significance to maintain normal levels of thyroid function and antibodies for elderly diabetic patients to control blood sugar and delay complications,improve cognition and emotion.This study aims to explore the general clinical characteristics of elderly patients with type 2 diabetes and the possible relationship between elderly diabetes and thyroid disease by studying the clinical data of patients with type 2 diabetes of different ages,so as to provide evidence-based medical basis for the development of treatment strategies for elderly diabetes complicated with thyroid abnormalities.Methods A total of 240 type 2 diabetes patients hospitalized in the department of Endocrinology in the Cadre Ward of our hospital from January 2019 to December 2019 were selected and divided into non-elderly type 2 diabetes group(age ≤60 years old)and elderly type2 diabetes group(age > 60 years old)according to age,including 113 cases in the non-elderly group and 127 cases in the elderly group.All biochemical parameters were determined by direct chemiluminescence method,the level of glycosylated hemoglobin was determined by high pressure liquid phase method,thyroid hormone and thyroid-related antibody levels were determined by electrochemiluminescence immunoassay.The statistical differences of each index between the two groups were compared and Pearson correlation analysis was conducted.In addition,the correlation between thyroid related indicators and clinical data was analyzed in elderly patients with type 2 diabetes.Results1.Compared with the non-elderly group,there were differences in age,course of disease,systolic blood pressure,diastolic blood pressure,fasting blood glucose,albumin,cystatin,total cholesterol,triglyceride,high-density lipoprotein cholesterol,very low density lipoprotein cholesterol,lipoprotein A and ferritin in the elderly group,and the differences were statistically significant(P<0.05);2.There was significant statistical difference in the number of patients with thyroid antibody positive between the elderly group and the non-elderly group(P<0.01),and the levels of TSH and TPO-Ab in the elderly group were higher than those in the non-elderly group(P<0.05);3.Pearson correlation linear analysis indicated that age was significantly positively correlated with course of disease and TSH(P<0.01),positively correlated with TPO-Ab(P<0.05),and fasting blood glucose was positively correlated with T4(P<0.05);4.ATG-Ab level was significantly positively correlated with systolic blood pressure in elderly patients with type 2 diabetes mellitusConclusion1.Compared with non-elderly patients with type 2 diabetes,elderly patients with type 2diabetes have significant differences in blood pressure,lipids,cystatin C and thyroid function,and are at greater risk of chronic complications;2.Elderly patients with type 2diabetes are at greater risk of hypothyroidism and abnormal antibodies as they age;3.There was a certain correlation between fasting blood glucose and T4 level;4.Abnormal thyroid antibodies may exacerbate systolic blood pressure fluctuations in elderly patients with type 2 diabetes.
