Ultrasound-guided Stellate Ganglion Block Accelerates Postoperative Gastrointestinal Function Recovery In Patients Undergoing Radical Gastrectomy For Gastric Cancer

Objective:To explore the effects of ultrasound-guided stellate ganglion block(SGB)on perioperative stress response,gastrointestinal hormones and postoperative gastrointestinal function recovery in patients undergoing laparoscopic radical gastrectomy for gastric cancer.Methods:Sixty American Society of Anesthesiologists physical classⅡ-Ⅲpatients,regardless of gender,aged 35-75 years old,with BMI18.5-26kg/m~2,undergoing elective laparoscopic radical gastrectomy.They were divided into two groups by random number table method:experimental group(S group,n=30)and control group(NS group,n=30).Group S was operated at the level of SGB at the right cervical spine 6under ultrasound guidance 15 minutes before induction of anesthesia,and injected 7ml0.5%ropivacaine.The NS group was injected with the same amount of normal saline.Ultrasound-guided SGB was conducted with 0.5%ropivocaine 7 ml at the right C6level in SGB group.The NS group was injected with 7ml normal saline in the same area.Peripheral venous blood samples were collected before SGB(T1),after surgery(T2),the second day after surgery(T3),and the sixth day after surgery(T4),and the levels of motitin(MOT),vasoactive intestinal peptide(VIP),cortisol(COR),and blood glucose(GLU)were determined.Intraoperative usage of sufentanil was recorded.The recovery rate of intestinal sounds at 36h,48h,60h,72h,84h and 96h after operation and the time of first exhaust were recorded.Result:Compared with NS group,there was no significant difference in the total amount of sufentanil supplementation between the two groups.Compared with NS group,the COR level,VIP level of S group at T2,T3 and T4 were lower than those of NS group(P<0.05),the MOT level were higher(P<0.05).Compared with NS group,Glu level at T2 and T3 was lower in S group(P<0.05).Compared with the NS group,the recovery rate of intestinal sounds in the S group at 36h,48h,60h,72h and 84h after surgery was higher(P<0.05),and the postoperative exhaust time was shorter(P<0.05).Conclusion:Ultrasound-guided SGB can reduce the postoperative stress level of patients with gastric cancer under laparoscopy,promote the recovery of the secretion level of gastrointestinal hormones,and accelerate the rapid recovery of postoperative gastrointestinal function.
