Clinical Observation Of Patients With Pneumoconiosis And Pulmonary Hypertension After Treatment By Comprehensive Intervention

Objective:To observe the clinical effect of the treatment by comprehensive intervention in patients with pneumoconiosis and pulmonary hypertension.Methods:60 cases of pneumoconiosis combined with pulmonary hypertension were randomly divided into control group,alveolar lavage group,comprehensive intervention group,20 cases in each group.The control group take symptomatic treatment（oxygen,anti-inflammatory,antispasmodic,antiasthmatic,expectorant）;Alveolar lavage group,the treatment of symptomatic treatment+saline whole lung lavage;Comprehensive intervention group,the treatment of symptomatic treatment+Salvia miltiorrhiza drug therapy+whole lung lavage with Low concentration of Salvia miltiorrhiza solution（per 500ml of saline added Salvia miltiorrhiza injection2ml）.6-minutes walking distance（6WMD）,pulmonary arterial systolic pressure（PASP）,quality of life CAT questionnaire score（CAT score）,and arterial oxygen saturation（Sa O2）were observed in the patients before treatment,1 months and 1year after treatment.Result:⑴There was no significant difference between the three groups in age,6WMD,PASP,CAT score,Sa O2 before the treatment（p all>0.05）.⑵Changes of 6WMD:In January,a year later,the control group of patients with 6WMD than before treatment increased,but the difference was not statistically significant（P all>0.05）.One month later,the 6WMD of the alveolar lavage group and the comprehensive intervention group were improved compared with those before treatment,the difference was statistically significant（P all<0.05）,there was no significant difference between the two groups.1 year later,the comprehensive intervention group 6WMD was significantly higher than that of the alveolar lavage group and the control group,the difference was statistically significant（P all<0.05）.⑶Comparison of PASP:1 month later,the three groups of SPAP were decreased,but there was no statistical difference between the control group and the control group before treatment（P>0.05）,The SPAP of the alveolar lavage group and the comprehensive intervention group were significantly lower than those before treatment（P all<0.05）.One year later,there was no significant difference in PASP between the control group and the control group,which was significantly increased at1 month（P<0.05）.Compared with the alveolar lavage group and control group,the indexes of comprehensive intervention group were significantly improved,and the difference was statistically significant（P<0.05）.⑷Comparison of CAT score:1 month later,the three groups of patients with CAT score than before treatment were improved,the difference was statistically significant（P all<0.05）.1 year later,the CAT score of the control group was higher than that of the treatment for 1 month,the CAT score of the alveolar lavage group and the comprehensive intervention group continued to decrease,compared with the control group,the difference was statistically significant（P all<0.05）,there was no significant difference in the CAT scores of alveolar lavage group compared with that in the same group（P>0.05）,The CAT score of comprehensive intervention group was better than that of the same group for 1 months（P<0.05）.⑸Changes of Sa O₂:1 month later,the three groups of patients with Sa O₂ than before treatment were improved,the difference was statistically significant（P all<0.05）,there was no significant difference between the three groups（P all>0.05）.1years later,there was no significant difference in Sa O2 between the control group and the control group（P>0.05）,the alveolar lavage group and comprehensive intervention group were all improved compared with before treatment（P all<0.05）,the comprehensive intervention group compared with the same group before treatment,1months of treatment were improved（P all<0.05）,compared with 1 year after the treatment,the difference was statistically significant（P<0.05）.Conclusion:Whole lung lavage and comprehensive intervention in the treatment of patients with pneumoconiosis and pulmonary hypertension,can improve the quality of life of the patients in the near future,delay the development of the disease,the recent effect is clear.The effect of the comprehensive intervention group was better than that of the alveolar lavage group.
