Objectives To know about the status of drinking and prevalence and incidence of fatty liver disease in the Jinchang Cohort,to reveal the effects of drinking on fatty liver disease and analysis of their relationship,and to explore factors influencing of prevalence and incidence of alcoholic fatty liver disease(AFLD),and provide a reference for cohort health drink and guidance for the prevention of fatty liver disease.Methods 46861 data were collected in baseline epidemiological surveys,clinical physical examinations and laboratory analysis from June 2011 to December 2014,including 28,445 male and18,416 female.The first phase of follow-up have been completed in 32,118 people,including 19,548 male and 12,570 female.We analyze the current situation of drinking and fatty liver disease based on the baseline data.In terms of follow-up data we analyze the fatty liver cumulative incidence condition for two years and could detected the effect of drinking on fatty liver disease pathogenesis.Logistic regression analysis was used to find the influencing fators for AFLD and Cox proportional hazards model analysis was used to find risk factors for AFLD.Results 1.The rate of drinking in Jinchang Chort was 24.85%.The rate of drinking in male is39.16%,involving 31.97% who were drinking and 7.20% who had drunk.the average age started drinking,average drinking time,frequency,drinking index,amount of alcohol on everyday,daily amount of alcohol was 24.84 years,22.56 years,2.91 times per week,770.18 g,73.28 g,81.62 g respectively,the rate of drink wine,red,beer,mixed drink in male was 25.93%,0.34%,6.10% and6.79% respectively.The rate of drinking in female was 2.98%,involving 24.65% who were drinking and 2.98% who had drunk.the average age started drinking,average drinking time,frequency,drinking index,amount of alcohol on everyday,daily amount of alcohol was 31.30 years,15.08 years,2.58 times per week,27.71 g,15.11 g,177.75 g respectively,the rate of drink wine,red,beer,mixed drink in male accounted for 0.93%,1.07%,0.40% and 0.35% respectively.The male mainly drink above 40 degree in wine.And most drinking frequency was once in a week,the mainly amount of alcohol in male and female was more than 50 g and less than 25 g.It is different for the types of alcohol in different age,occupation,marital status,education level,income of the family.2.The prevalence of fatty liver in Jinchang Chort was 22.56%,27.81% in male and 14.46% in female.The prevalence of nonalcoholic fatty liver disease was 17.05%,17.01% in male and 14.04%in female respectively and the prevalence of alcoholic fatty liver disease was 3.32%,5.44% in male and 0.07% in female respectively.The accumulative incidence of fatty liver disease in two years was 7.90%,8.07% in male and 7.64% in female.The accumulative incidence of nonalcoholic fatty liver disease was 6.67%,6.19% in male and 7.42% in female.The accumulative incidence of alcoholic fatty liver incidence was 0.48%,0.78% in male and 0.02% in female.3.Drinking was associated with fatty liver disease,the prevalence of fatty liver in different state of drinking,drinking types,drinking amount,drinking time,the amount of daily alcohol were different.When the amount of alcohol less than 25 g every time is a protective factor in fatty liver disease and nonalcoholic fatty liver disease.When drinking time less than 10 years and drinking frequency within 2 times per week and each time less than 50 g alcohol is the protection of fatty liver disease and nonalcoholic fatty liver disease.The incidence of fatty liver disease and nonalcoholic fatty liver disease in different state of drinking,drinking type,amount of alcohol drinking index,drinking time are different,there were no significant statistically difference in the amount of alcohol.4.When control in body mass index,liver function and glucose levels,only in normal people with the increase of the amount of daily alcohol and the prevalence of nonalcoholic fatty liver disease have significantly different.At the same time high body mass index,liver function and blood glucose levels are risk factors affecting the fatty liver disease.The amount of alcohol of nonalcoholic fatty liver disease in each group have no significant difference,and high body mass index,abnormal liver function,high blood glucose level are risk factors for alcoholic fatty liver disease.The rise of GGT caused by drinking is the reason lead to fatty liver disease.5.Multiple factors logistic regression analysis results showed that 45-75 age groups,marriage and death of spouse,smoking index of more than 600,drinking indexes,eating more than 350 g meat a week,high ALT,GGT,BMI and FBG,abnormal liver function is the influencing factors of alcoholic fatty liver.high education,smoking index of less than 150 were the protection of alcoholic fatty liver disease.cox regression analysis results showed that alcohol,GGT,BMI are the risk of alcoholic fatty liver disease,and more vegetables and fruits were protection factors of alcoholic fatty liver disease.Conclusions 1.The drinking ratio in Jinchang cohort at a medium level,male is the main drinking people.The main drinking type was wine,while women was red.Drinking types were affected by age,occupation,education degree,marital status and family income.2.The prevalence and incidence of fatty liver disease in Jinchang cohort were at a medium level,nonalcoholic fatty liver disease and alcoholic fatty liver disease incidence rate were low,and the incidence of male were higher than female.3.A small amount of alcohol is the protection factor of nonalcoholic fatty liver disease,when the alcohol dose rised to the dose of alcoholic fatty liver disease,the prevalence would not increase with the increase of the dose.Alcohol has no effect on incidence of nonalcoholic fatty liver disease,high GGT is the leading cause of fatty liver disease of liver enzymes.4.Middle age groups,married and widowed people,large amount of alcohol drinking,high AST,GGT,BMI,FBG levels,abnormal liver function were risk factors of alcoholic fatty liver disease,a small amount of smoking and high education were the risk factor in incidence of alcoholic fatty liver disease.High age group,eat more vegetables and fruits were the protection factors of alcoholic fatty liver.
