| Objects: With the increasing incidence of diabetes mellitus,the diabetic nephropathy has become the main cause of end-stage nephropathy.Shear wave ultrasonic elastography technology has been confirmed that has certain scientific basis in the evaluation of tissue fibrosis degree.This study was to research the correlation between renal parenchyma elasticity of shear wave and clinical staging indexes of diabetic nephropathy.Explore the clinical significance of that use renal parenchyma shear wave elastography technology to evaluate the clinical significance of the severity of diabetic nephropathy and diagnosis.Methods: Strict criteria for the participants,excluding other factors that may cause kidney disease change,such as primary resistant hypertension,systemic disease.Selected 59 cases of diabetic patients as observation group,all diabetic patients underwent urine protein quantification and hematuria laboratory index test,and according to the level of Urinary Albumin Excretion Rate can be divided into simple diabetes group,the early DN group and clinical DN group.Select 20 cases of healthy physical examination as a control at the same time.The real-time shear wave elastography quantitative technique was used to detect the elasticity of renal parenchyma of all subjects,the blood flow parameters of renal arteries were measured by color doppler.The changes of blood flow parameters,young’s modulus of renal parenchyma and blood flow parameters of clinical indexes in each group were observed.Results: Young’s modulus of renal parenchyma in each group was compared and found that early DN and clinical DN group of young’s modulus value of renal parenchyma is significantly higher than the control group,and clinical DN group is higher than the early DN group,the difference is statistically significant(P <0.05),while simple diabetes group compared with normal control group,there was no statistically significant difference(P >0.05).The correlation between young’s modulus of renal parenchymal excretion,creatinine,urea,and uric acid,and the changes of young’s modulus of renal parenchymal excretion and creatinine were more significant(r=0.70,r=0.62).Move each kidney artery contrast found at all levels,main renal artery and segmental arteries between PSV,EDV,leaf EDV gradually reduce the trend with the increase of installment,leaf artery between PSV does not change significantly with the increase of installment,clinical DN groups and all levels of arterial RI early DN group were higher than that of control group,the differences were statistically significant(P<0.05),while clinical DN group compared with early DN group,only the main renal artery and arterial RI difference was statistically significant between leaf,two groups of artery RI was no difference(P >0.05).Comparing the young’s modulus value of renal parenchyma with the blood flow parameters of renal arteries at all levels,it was found that YM value was negatively correlated with PSV of main renal artery and segmental artery EDV(r=-0.7 、-0.74),and positively correlated with RI of main renal artery,segmental artery and interloba artery(r=0.7 、 0.69 、 0.66),and the difference was statistically significant(P <0.05).Conclusion: Young’s modulus of renal parenchyma is correlated with laboratory indexes of clinical stages of diabetic nephropathy,and the quantitative technique of shear wave elastography of renal parenchyma has certain reference significance for clinical stages of diabetic nephropathy. |
