| Object Through analyzing the relationship between the expression of EMPs and total CSVD burden score in patients in cerebral small vessel disease(CSVD)to explore the role of plasma endothelial microparticles(EMPs)as a biomarker of endothelial cell dysfunction and the possibility of monitoring the severity of CSVD pathological damage as well as its relationship with cognitive impairment.Methods Patients hospitalized in the Department of Neurology,Zhongnan Hospital of Wuhan University,from May 2017 to May 2018,were enrolled.We assessed the cognitive function of the patients,collected the data of clinical history and laboratory examination results and detected the concentration of EMPs(by flow cytometry)and inflammatory factors(TNF-α,IL1-β).Then,we analyzed the correlation between different phenotypes of EMPs(CD31+/42b-,CD62E+)and their concentrations with total CSVD burden score and cognitive function,and analyzed the relationship between inflammatory factors and EMPs and their roles in CSVD injury at the same time.Results A total of 104 patients,including 58 females and 46 males,were enrolled in the study.(1)Univariate logistic regression analysis showed that age was positively correlated with the risk of CSVD(p < 0.05);(2)Compared with the normal group,the concentration of CD31+/CD42b-EMP and CD62E+EMPs in group 1,group 2 and group 3-4 were significantly higher(P < 0.05).The level of circulating EMPs was positively correlated with the total CSVD burden(p < 0.05,r = 1.007 or 1.012).(3)Compared with control group,the concentration of TNF-α and IL-1β in patients with CSVD significantly increased(P < 0.05).The correlation analysis between the concentration of inflammatory factor and EMPs showed that there was a positive correlation between IL-1 β and CD31 / CD42b-EMPs(P < 0.01,r = 0.656).The correlation coefficient between IL-1 β and CD62E+EMPs was 0.263(P < 0.007).The correlation coefficient between TNF-α and CD31+/CD42b-EMPs was 0.475,P <0.001;The correlation coefficient between TNF-α and CD62E+EMPs was 0.287,P <0.003.(4)The scores of Mo CA、RAVLT、DST、VFT and DSST gradually decreased with the increase of total CSVD score,while the time of TMT-A/B was negatively correlated with the total CSVD score.Circulating EMPs concentration was negatively correlated with cognitive function.Besides,IL-1 β was negatively correlated with MMSE,while there was no correlation between TNF-α and cognitive function.Conclusion Age is independent risk factor of CSVD.The concentration of EMPs increased in CSVD,and the degree of increase was positively correlated with the inflammation.The level of EMPs in CSVD patients were positively correlated with the severity of CSVD.As the severity of CSVD increases,cognitive function gradually decreased. |
