| Objective To explore the application value of three-dimensional turbo field echo(3D-TFE)and three-dimensional ultrashort echo time(3D-UTE)in pulmonary and tracheobronchial anomalies which were associated with congenital heart disease.Methods1.Five bama miniature pigs were enrolled in our study and performed MRI 3D-TFE,3DUTE airway scanning and chest CT scan.Radiologists evaluated the feasibility of 3D-TFE and 3D-UTE in showing tracheobronchial tree.(2)Pneumonia animal models in five bama miniature pigs were made, and underwent MRI 3D-UTE pulmonary scanning and chest CT scan.Compared to CT results, radiologists evaluated the feasibility of 3D-UTE in showing pulmonary lesions.2.Sixty-two consecutive children were enrolled, who were diagnosed congenital heart disease by echocardiography. All children underwent MR examination, including CE-MRA,3D b-TFE,and 3D-TFE sequence.Image quality was objectively evaluated including signal-tonoise ratio, signal intensity and tracheal boundary clarity. Two independent radiologists subjectively evaluated image quality and image noise using a 5-point scale, and analyze the value of 3D-TFE in assessing tracheobronchial anomalies of congenital heart disease.3.We recruited 24 children who were confirmed congenital heart disease by echocardiography. They were referred to cardiac MRI and chest CT examination respectively.3D-UTE sequence was performed to assess tracheobronchial tree, detect tracheobronchial and pulmonary anomalies. Two radiologists in blinded way scored 3D-UTE image quality using 5-point scale independently and analyzed the value of 3D-UTE in assessing tracheobronchial and pulmonary anomalies of congenital heart disease.Results1.3D-TFE and 3D-UTEclearly showed tracheobronchial tree of bama miniature pigs;3DUTE could showed pulmonary anomalies of bama miniature pigs.2.The SNR,signal intensity, tracheal boundary clarity and image quality scores of 3DTFE in trachea was higher than that of CE-MRA and3 D b-TFE.3.The sensitivity, specificity, positive predictive value and negative predictive value of 3D-UTE in detecting tracheobronchial anomalies were100%,88.89%,93.75% and 100% respectively. The results of 3D-UTE pulmonary imaging are consistent with CT images, andthe diagnostic accuracy was 100%.The image quality and diagnostic confidence of 3D-UTE were good. Conclusion The image quality of 3D-TFE sequence for demonstrating tracheobronchial tree is superior to the 3D b-TFE and CE-MRA sequences.3D-UTE sequence could clearly show tracheobronchial structure, accurately detect tracheobronchial and pulmonary anomalies. As essential auxiliary sequences for cardiac magnetic resonance imaging,3D-TFE and 3D-UTE can detect tracheobronchial and pulmonary abnormalities which were associated with congenital heart diseases, and then supply helpful information for preoperative strategies and postoperative follow-up. |
