ObjectivesThe cohort study design was used to collect the raw data of surrogate markers and endpoints during the hospitalization and follow-up period of patients with chronic heart failure in TCM hospitals.Based on Cox regression model and Logistic regression model,the surrogate markers for TCM clinical efficacy evaluation of chronic heart failure were selected.MethodsUse medical records and follow-up surveys.Produced a questionnaire for the cohort study of patients with chronic heart failure in Chinese medicine hospitals.Through the medical record information management system,three hospitals of Shandong University of Traditional Chinese Medicine,Weifang Traditional Chinese Medicine Hospital and Rizhao Traditional Chinese Medicine Hospital were selected.They were admitted to hospital from January 1,2012 to December 31,2017.The hospital was diagnosed with chronic heart failure.Patient.The general data of the patients were collected,and the surrogate indicators and endpoint events occurred during the hospitalization period and during the follow-up period.The follow-up period was until July 31,2018.Epidata 3.1 software was used to establish a cohort study database for patients with chronic heart failure and data were entered.Exported to Excel 2003 and SPSS17.0 software package,statistics related substitution indicators and endpoint events,and then use Cox regression model to calculate the OR value of the correlation between surrogate markers and endpoint events.Logisticregression model calculates surrogate markers and TCM interventions.The OR value of correlation is used to construct a surrogate index for the evaluation of clinical efficacy of TCM in chronic heart failure.Result1.The occurrence of endpoint events The frequency of endpoint events during hospitalization and follow-up was followed by heart failure re-admission(49.02%),cardiac death(28.37%),death due to death(11.52%),and acute coronary syndrome(3.93%).),severe arrhythmia(2.25%).2.Correlation analysis of surrogate markers and endpoints The surrogate markers predict the intensity of endpoint events in the order of NYHA cardiac function(OR=2.117),amino-terminal B-type natriuretic peptide precursor(OR=1.019),age(OR=1.012).),left ventricular ejection fraction(OR=0.989),hemoglobin(OR=0.977),low-density lipoprotein cholesterol(OR=0.694).3.Correlation analysis between surrogate index and TCM intervention The sensitivity of surrogate markers for TCM treatment was: NYHA cardiac function grading(OR=1.561)and left ventricular ejection fraction(OR=1.160)were the relevant factors for TCM intervention in chronic heart failure.ConclusionThe main surrogate index for clinical efficacy evaluation of chronic heart failure in Chinese medicine is NYHA cardiac function grading.The secondary replacement indicators are amino terminal B-type natriuretic peptide precursor,left ventricular ejection fraction,and hemoglobin.
