| Objective: to investigate the safety of hyperthermic intraperitoneal chemotherapy for gastrointestinal peritoneal metastatic carcinoma.Methods: a retrospective analysis was performed on 41 patients with gastrointestinal malignancy with peritoneal metastasis admitted to the cancer hospital of guangxi medical university from March 2019 to March 2020.There were 24 males and 17 females,with an average age of 49.87±15.4 years.Hyperthermic intraperiteal chemotherapy(HIPEC)combined with systemic chemotherapy was defined as group A,with A total of 19 patients.Group B was treated with systemic chemotherapy alone,with a total of 22 cases.General information was collected for all enrolled patients.Body temperature,pulse,respiration and blood pressure were collected at the beginning,30 minutes and the end of group A HIPEC.The incidence of anemia,leukopenia,granulocytopenia,liver and kidney dysfunction,nausea,vomiting,abdominal pain,abdominal distension,intestinal obstruction,abdominal infection and death after treatment in group A and Group B.Changes of peripheral blood tumor markers CEA,CA125 and CA199 in patients in group A and group B before and after treatment.This study was approved by the hospital ethics committee,and the patient and his/her family signed the informed consent for abdominal hyperthermia and chemotherapy.Results:The body temperature,pulse,respiration,systolic and diastolic blood pressure were compared at the beginning,30 minutes and the end of HIPEC in group A.As a result,the body temperature(F = 0.53,P = 0.59),pulse(F = 0.97,P = 0.39),respiratory(F = 1.45,P = 0.24),systolic blood pressure(F= 0.14,P = 0.87),diastolic blood pressure(F = 0.09,P = 0.92)were greater than0.05,there was no statistically significant difference.Adverse reactions: the incidence of grade 1 and grade 2-4 related adverse reactions before and after treatment in group A and group B were compared.Chi-square test was used for statistical analysis.The results showed that the incidence of adverse reactions in group A and group B was greater than 0.05 before and after treatment,and the difference was not statistically significant.The incidence rates of abdominal pain,abdominal distension,intestinal obstruction,intestinal fistula,intestinal obstruction,abdominal infection and death complications were compared between group A and group B before and after treatment.Chi-square test was used for statistical analysis,and the results showed that the p values of all complications before and after treatment in group A and group B were all greater than 0.05,with no statistically significant difference.The changes of CEA,CA125 and CA199 in peripheral blood serum were compared before and after treatment in group A.The statistical method of paired sample t test was used.Results: serum CEA,CA125 and CA199 were(95.14±335.53),(63.81±83.03)and(95.66±279.65)in group A before treatment,and(92.87±351.57),(28.83±31.56)and(165.67±383.42)in group A before treatment.CA125 was lower in group A before and after treatment than before treatment(P<0.05),and the difference was statistically significant.CEA decreased before and after treatment(P > 0.05),and the difference was not statistically significant.CA199 was higher than that before surgery(P > 0.05),with no statistically significant difference.The changes of CEA,CA125 and CA199 in peripheral blood serum were compared before and after treatment in group B.Before treatment,CEA(12.45±15.23)、CA125(75.11±131.03)、CA199(194.72±369.10),CEA(26.01±47.98)、CA125(46.03±63.60)、CA199(233.55±416.95)in group B were analyzed by paired sample t test.CEA and CA125 decreased before and after treatment(P > 0.05),and the difference was not statistically significant.CA199 was higher than that before surgery(P > 0.05),with no statistically significant difference.Conclusion: the treatment of gastrointestinal peritoneal metastatic carcinoma is safe and has no obvious effect on the patient’s body temperature,pulse,respiration,systolic blood pressure and diastolic blood pressure.Compared with systemic chemotherapy alone,the incidence of adverse reactions did not increase significantly.CA125 is expected to be an auxiliary index to evaluate the efficacy of intraperitoneal high temperature chemotherapy for peritoneal metastatic carcinoma from gastrointestinal tract. |
