| Objective:With the increasing prevalence of obesity in children in China,the hidden penis is often confused with the buried penis caused by obesity.This study provides a new way of clinical differentiation between concealed penis and buried penis by measuring the angle of abdominal wall penis of concealed penis,buried penis and phimosis.Method:From January 2018 to January 2020,19 cases of concealed penis,7 cases of buried penis and 19 cases of phimosis were selected for diagnosis and treatment in our hospital.Height and weight were measured,body mass index BMI was calculated,and the degree of obesity of children was judged,which were recorded as normal,overweight and obesity.Take pictures of the lower abdomen and penis of the side of standing position and the side of prone position,process the pictures with Adobe Photoshop,respectively measure the angle of the abdominal wall penis in two directions,record it as the angle of the abdominal wall penis(standing position)and the angle of the abdominal wall penis(prone position),and improve the measurement method of the angle of the abdominal wall penis,so as to pass through the measurement vertex and be perpendicular to the measurement plane The angle of the penis on the abdominal wall is divided into two parts: the angle of the penis on the side near the penis and the angle of the abdominal wall on the side near the abdominal wall.The angle measurement results of three angles and six planes of three diseases were statistically analyzed,and then the statistical differences were compared according to the normal weight,overweight or obesity,combined with the findings and pathology during operation,to explore the clinical application value of different planes of three angles for concealed penis and buried penis.Result:1.Angle of abdominal wall penis: there was no statistical difference between the three positions,whether standing or supine.2.Penis angle: in standing position,regardless of the overall or similar degree of obesity,there is statistical difference between concealed penis and buried penis or phimosis,but there is no statistical difference between buried penis and phimosis;there was no statistical difference among the three in the supine position.3.Abdominal wall angle: in standing position,there is statistical difference between buried penis and concealed penis or phimosis,but there is no statistical difference among the three in the comparison of overweight or above;in lying position,there is only statistical difference between concealed penis and phimosis,but there is no statistical difference between the two in the comparison of similar obesity degree.4.Histopathological observation: the superficial layer of concealed penis specimen can be seen as fibrous tissue,which is dense and disordered,and the deep layer can be seen as adipose tissue;the deeper layer of buried penis specimen can be seen as fibrous tissue,which is loose and regular,and the superficial layer can be seen as adipose tissue.Conclusion:1.The measurement of standing position and lying position of the angle of the penis on the abdominal wall does not help to distinguish the concealed penis from the buried penis;2.The angle of the penis(standing position)and the angle of the abdominal wall(standing position)help to distinguish the concealed penis from the buried penis,especially for overweight or obese children,the role of the angle of the penis is more obvious;3.By comparing the angle of the penis and the angle of the abdominal wall of the concealed penis,the buried penis and the phimosis,combined with pathology It also proves the etiology of concealed penis and buried penis,and provides a new way to distinguish them in clinical practice. |
