| Objective: To compare and analyze the clinical characteristics and prognosis of patients with SSHL(Sudden Sensorineural Hearing Loss,SSHL)with endolymphatic hydrops and non-endolymphatic hydrops.Methods: The clinical data of 63 patients with unilateral SSHL diagnosed in the first affiliated Hospital of Fujian Medical University from August 2009 to December 2019 were analyzed retrospectively.The patients were divided into hydrocephalus group and non-hydrocephalus group according to the results of inner ear gadolinium radiography.The clinical manifestations were compared with the follow-up period from 3 months to 10 years,with an average follow-up time of 5.7 ±3.12 years.The outcome was collected by telephone,outpatient follow-up and questionnaire.Results:1.There was significant difference in the frequency of hearing loss between the hydrocephalus group and the non-hydrocephalus group(P=0.03<0.05).2.Through follow-up,the number of people who were finally diagnosed with Meniere’s disease(Meniere’s Disease,MD)was statistically significant between the two groups(P=0.001<0.05).The number of patients with hydrops was significantly higher than that of patients without hydronephrosis.3.There was no significant difference in age(P=0.079),sex(P = 0.603)and side(P = 0.603)between the two groups(P >0.05).4.In terms of clinical features,there was no significant difference in the degree of hearing loss(P = 0.141),vertigo(P = 0.210)and tinnitus(P = 0.355)between the two groups(P > 0.05).5.To observe the changes of inner ear in patients with SSHL after intratympanic(IT)gadolinium injection or intravenous(IV)gadolinium injection,and there was no significant difference between the two groups.(P = 0.556 > 0.05).Conclusion:The probability of late diagnosis of MD in SSHL patients with endolymphatic hydrops is significantly higher than that in non-endolymphatic hydrops patients.It is necessary to be aware that sudden hearing loss with endolymphatic hydrops may be the early manifestation of MD. |
