The Clinical Evaluation Of Glaucoma Miniature Drainage Device Implantation Combined With Phacoemulsification

Objective.To analyze and investigate the validation and security of Ex-PRESS glaucoma miniature drainage device implantation combined with phacoemulsification(Phaco)for the surgical treatment of primary glaucoma.Methods.The retrospective analysis contained 43 eyes of 35 patients suffered from primary glaucoma(including primary open-angle glaucoma and primary angle-close glaucoma)and treated with Ex-PRESS glaucoma miniature device implantation combined with Phaco from February 2019 to November 2019 in our hospital.The intraocular pressure(IOP),best corrected visual acuity(BCVA),anterior chamber depth,filter bleb,the number of post-operative anti-glaucoma medications,successful operation rate,reoperation rate,post-operative complications were calendared before operation and 1d,1w,1mon,3mon post-operation.Statistical analysis of the above observation items of pre-operation and post-operation was procedured in order to compare the effectiveness and security among different groups.Results.The post-operative successful operation rate at 1d,1w,1mon,3mon was95.3%、97.7%、83.7%、90.7%.Post-operative l OP at each review time decreased significantly when compared with preoperation(P<0.05).The species of anti-glaucoma medications reduced meaningfully after surgery(X~~2=47.996,P<0.05).Statistically difference was proved in BCVA between the group of pre-operation and groups of post-operation(P<0.05)except the group of 1d post-operation(Z=-1.933,P=0.053).The complications mainly included temporary corneal edema in 9 eyes,transient low IOP in 3 eyes.13 patients suffered from after-cataract received Nd:YAG laser capsulotomy of lens.Non-functional bleb occurred in 3 eyes,and 1 another eye received reoperation at 3mon after surgery because of high intraocular pressure.There was no serious complication occurred.Conclusion.The Ex-PRESS glaucoma miniature drainage device implantation combined with phaco is proved to be an effective and safe treatmental method to primary glaucoma complicated by cataract.The one stage combination surgery can reduce IOP and improve BCVA simultaneously with less complications and without severe complications.
