| ObjectiveA big data,multi-center clinical study involving more than 120 hospitals nationwide has completed in China,and a new gastric cancer screening scoring system was proposed in 2017 based on this study.We prospectively collected epidemiological data and performed serological tests to verify the value of the new scoring system in high-risk populations in in the island of Fujian.At the same time,we analyzed the epidemiological data of those people who were diagnosed with gastric cancer and precancerous lesions,in order to find out the characteristic high risk factors for Fujian island population.MethodsSurvey of risk factors for gastric cancer and precancerous diseases in the high-risk population in the island of Fujian.From April to June 2019,we screened the population from the age of 40 to 75 in the Nanri Island,Putian.According to the selection criteria,a total of 1423 people were included to the project.We collected the epidemiological data of all study subjects.All subjects were also tested for gastrin 17,PGI,and PG II on an empty stomach.After that gastroscopy was performed,and magnifying endoscopy and biopsy of suspected positive cases followed.Risk factors for gastric cancer and precancerous lesions were analyzed as independent variables,and statistical methods such as logistic regression and chi-square test were used.Evaluation of the value of a new scoring system for gastric cancer screening for gastric cancer screening in high-risk populations in the island of Fujian.According to the new scoring system for gastric cancer screening,the high risk population of gastric carcinoma are divided into three groups: low-risk group,moderate risk group,and high-risk group.The detection rates of gastric cancer and the detection rates of gastric cancer and precancerous lesions were compared in the three groups to verify the value of the new scoring system in high-risk populations in the island of Fujian.Results1.Survey of risk factors for gastric cancer and precancerous diseases in the high-risk population in the island of Fujian.The variables with P <0.1 in the univariate logistic regression analysis were included as candidate factors in the multivariate analysis,including gender,age,pickled food intake,fresh vegetable /fruit intake,smoking,drinking,and tea-drinking.After multiple logistic regression analysis,Male(P=0.009,OR=1.729),age(P=0.002,OR=1.729),pickled food intake(P=0.029,OR=1.696),tea-drinking(P=0.002,OR=1.874)are risk factors of gastric cancer and precancerous lesions.Fresh vegetable and fruit intake(P<0.001,OR=0.029)are protective factors for gastric cancer and precancerous lesions.2.Evaluation of the value of a new scoring system for gastric cancer screening in high-risk populations in the island of Fujian.A total of 19 cases of gastric cancers were detected in 1423 patients,and the detection rate of gastric cancer was 1.34%.The detection rates of gastric cancer in the low-risk group,intermediate-risk group,and high-risk group were 0.87%(9/1025),1.76%(6/341),and 7.02%(4/57).The paired comparison were performed.We found that there was a statistical difference between the low-risk group and the high-intermediate-risk group(P = 0.015),and the difference between the low-risk group and the high-risk group was also statistically significant(P = 0.003).There were no statistical differences between the low-risk group and the moderate risk group,and between the moderate risk group and the high-risk group.A total of 106 cases of gastric cancer and precancerous lesions were detected in all patients,with a detection rate of 7.45%.The detection rates in the low-risk group,moderate risk group,and high-risk group were 7.07%(73/1025),6.74%(26/341)and12.28%(7/57).There were no significant differences between the three groups.ConclusionThrough epidemiological investigation,we found that the risk factors of gastric cancer and precancerous lesions in the population of the island of Fujian were male,age,intake of pickled foods,and tea drinking.While intake of fruits and vegetables was protective factors.Identifying high-risk factors for gastric cancer in the island of Fujian is beneficial for intervention and comprehensive prevention.The new scoring system for gastric cancer screening that used for screening high-risk populations of gastric cancer in the islands of Fujian is economical and effective.With early diagnosis and treatment of early gastric cancer,patients can obtain a better prognosis. |
