Time-zero Biopsy Combined With Morphology To Assess The Quality Of Donation After Citizen Death Kidney

Objective:To explore the relationship between the pathological characteristics of DCD donor kidney and renal function recovery after receptor operation,and to provide experimental evidence for renal selection in clinical renal transplantation.Methods:(1)The medical records of renal transplantation in the second affiliated Hospital of Hainan Medical University from January 2018 to January 2019 were collected by Hospital Information System.According to the Banff criteria,the pathology of time-zero biopsy of donor kidney was scored.At the same time, according to the self-made score table of gross morphological changes,the gross morphological change of donor kidney after repair was scored.(2)To observe the occurrence of creatinine and delayed graft fimction(DGF)in recipients,and to make a retrospective analysis.Results:A total of 58 cases of qualified cases were included in this study,of which 14cases were DGF,and the incidence rate of DGF is 24.1%.The sum of the scores of histological scores of nephropathy was 1-8,low group(<3 points)has 26 cases,and32 cases with high group(≥3 points).The gross morphological change scores were distributed among 0-4 points.(1)Renal interstitial fibrosis were significantly correlated with creatinine on the 7th day after receptor operation(P=0.006).(2)There was a significant correlation between the total histopathological score and the occurrence of serum creatinine level and DGF in the 3rd,4th and 7th days after operation,The incidence of high group and the low group DGF are differences between groups(χ~2=4.085;P=0.043).(3)There was a significant correlation between renal texture and the 7th day of creatinine after receptor operation(P=0.012).(4)The total score of general morphological changes was correlated with the level of blood creatinine and histopathological score of the 3rd day,4th day and 7th day after operation(p<0.05).Conclusion:(1)Renal interstitial fibrosis and renal interstitial inflammation are related to early creatinine levels after receptor transplantation,which can be used as an index for reference for the evaluation of DCD donor kidney quality.(2)BANFF histopathological score can be used as a predictor of DGF after receptor operation,the higher the score,the greater the likelihood of DGF occurring after receptor operation.(3)The kidney texture can be used as an index for reference for the evaluation of DCD donor kidney quality.
