Clinical Study On The Treatment Of Cerebral Watershed Infarction With Qi-deficiency And Blood-stasis By Tonifying Qi And Activating Blood

Objective:To observe the clinical efficacy of buqi huoxue prescription in the treatment of qi-deficiency and blood-stasis type cerebral watershed infarction,and preliminarily explore its mechanism of action,so as to further enrich the thoughts of TCM diagnosis and treatment of cerebral watershed infarction,so as to benefit the clinic.Methods:Sixty-six patients with cerebral watershed infarction of qi deficiency and blood stasis type who were hospitalized in the department of encephalopathy,weifang hospital of traditional Chinese medicine from June 2018 to December 2019 were randomly divided into the treatment group and the control group according to the sequence of admission to the hospital,with33 cases in each group.The control group received routine neurological treatment according to the 2018 guidelines for the diagnosis and treatment of acute ischemic stroke in China: aspirin enteric-coated tablets 0.1g qd Po;Atorvastatin calcium tablets 20mg qn Po;Edaravone injection 30mg into 0.9%NS100ml by intravenous drip bid;For injection,2g of olacetam was injected into 0.9% NS100ml of qd.On the basis of it,the treatment group took tonic qi and invigorating blood prescription(the composition was as follows:Raw Astragalus 60g,Codonopsis pilosula 15g,peach kernel 6g,safflower 6g,red peony root 15 g,angelica sinensis 15 g,ligusticum chuanxiong 9g,earthworm 6g,2pieces of centipede,ox genu 10g,processed licorice 6g)200ml each time,and took it warm in the morning and evening every day.Both groups were evaluated after 14 days of treatment.The general information of the two groups(age,gender,disease course,past history,classification),TCM syndrome score before and after treatment and the efficacy of TCM syndrome,NIHSS score and the efficacy of neurological impairment degree,daily living ability(improved Barthel index)score and safety indexes were compared.SPSS22.0 statistical software was used to analyze the experimental data.The measurement data were expressed by(x±s).Those conforming to the normal distribution were tested by t test,those failing to conform to the normaldistribution were tested by the rank sum test,and the counting data were tested by Person2 test.P is a bilateral test,P < 0.05 means the difference is statistically significant,and P < 0.01 means the difference is statistically significant.Results:During the study,6 cases were dropped for different reasons,and 30 cases were eventually completed in each of the two groups.There was no statistical difference in general data between the treatment group and the control group.The TCM syndrome score,NIHSS score and daily living ability were significantly improved compared with those before treatment,and the treatment group was better than the control group.The total effective rate was 96.66% in the treatment group and 73.33% in the control group.By rank-sum test,p<0.05,the difference was statistically significant.The total effective rate was 100% in the treatment group and 90.00% in the control group.By rank-sum test,p < 0.01,the difference was statistically significant.The treatment group was superior to the control group.No significant adverse reactions were found in the safety indicators of the two groups.Conclusion:Tonic qi and invigorating blood formula can reduce the TCM syndrome score and NIHSS score of patients,improve the Barthel index,reduce the clinical symptoms of patients,improve the degree of neurological impairment and daily living ability,and improve the quality of life.At the same time,the application of this prescription is relatively safe,without obvious liver and kidney function damage and toxic side effects.
